| Objective: To observe the clinical effect of moxibustion combined with small intestinal decompression tube in the treatment of right colon cancer combined with intestinal obstruction,and to evaluate the efficacy and safety of right colon cancer combined with intestinal obstruction through this treatment plan,in order to provide scientific basis for the treatment of right colon cancer combined with small intestinal decompression tube.Method: Retrospective analysis of 60 patients who met the diagnostic criteria for right colon cancer combined with intestinal obstruction and were not undergoing emergency surgery in the General Surgery Department of Jiangxi University of Traditional Chinese Medicine Affiliated Hospital(Jiangxi Provincial Hospital of Traditional Chinese Medicine)and the General Surgery Department of Jiangxi Provincial People’s Hospital.All patients had no contraindications for small intestine decompression tube placement and underwent digital subtraction angiography(DSA)guided placement of a nasal small intestine decompression tube,Divide the subjects into an experimental group and a control group,with 30 cases in each group.The control group received routine Western medicine treatment and placement of a nasal small intestine decompression tube.The experimental group received moxibustion therapy on the basis of the control group;After treatment,compare the clinical symptoms improvement,gastrointestinal function recovery,abdominal circumference,abdominal pressure,inflammatory indicators [white blood cell count(WBC),C-reactive protein(CRP),procalcitonin(PCT),serum amylase protein A(SSA),neutrophil granulocyte(N)] Gastrointestinal hormone [Gastrin(GAS),motilin(MTL),vasoactive peptide(VIP)],intestinal mucosal barrier permeability index [D-lactic acid,endotoxin,diamine oxidase(DAO)],the impact of intestinal cleanliness seen during the operation,the change of intestinal decompression tube drainage flow and its improvement methods.Results:1.Moxibustion combined with small intestinal decompression tube in the treatment of right colon cancer complicated with ilobstruction has obvious curative effect.After treatment,the gastrointestinal function recovery time and postoperative hospital stay in the experimental group are shorter than those in the control group(P<0.05).2.In terms of clinical concomitant symptoms,the disappearance time of abdominal pain,abdominal distension,nausea and vomiting was shorter than that of the control group(P<0.05),but there was no significant difference in the recovery time of abdominal plain film between the two groups(P>0.05).3.After treatment,abdominal circumference and abdominal pressure in 2 groups were decreased compared with that before treatment(P<0.05),there was no significant difference between the experimental group and the control group(P>0.05).4.There was no statistical significance in the comparison results of hemoglobin,albumin,potassium ion and sodium ion between the experimental group and the control group(P>0.05),there were significant differences between the two groups after treatment compared with before treatment(P<0.05).5.Inflammatory(sexual)indicators:The levels of WBC,CRP,PCT,SAA and N in the experimental group before and after treatment were statistically significant(P<0.01);The levels of WBC,CRP,PCT,SAA and N in the control group were significantly different before and after treatment(P> 0.01).6.Gastrointestinal hormone:Serum GAS levels in the experimental group and the control group were significantly different on day 3 and 7 after treatment(P<0.01);There were statistically significant differences in serum GAS levels between time and group(interaction)(P<0.05);The serum MLT levels of the experimental group and the control group were significantly different on day 3 and day 7 after treatment(P<0.01);Differences in serum MLT levels were statistically significant by comparison of time,group,and interaction(time and composition)(P<0.05);The VIP level of the experimental group and the control group was significantly different on the 7th day after treatment(P<0.05).7.Intestinal mucosal barrier permeability indicators:The D-lactic acid in the experimental group and the control group was statistically significant on day 7 after treatment(P<0.05)and no difference at other time points(P>0.05);Serum endotoxin levels were statistically significant on day 1and day 7 after treatment(P<0.05),DAO was significantly different at 3 and 7 days after treatment(P<0.05).8.Intestinal cleanliness score: There was no statistical significance in intestinal cleanliness between the two groups during the operation(P>0.05).9.Small intestinal decompression tube drainage volume: The drainage volume of small intestinal decompression tube was compared at the first,third and fifth days after treatment,and there were differences in different periods(P<0.01);There was no statistical significance in intestinal decompression tube drainage between the two groups(P>0.05);There were differences in the trend of intestinal decompression tube drainage between the two groups(P< 0.01).10.There was statistical significance in the comparison of clinical effective rate between the experimental group and the control group(P<0.05),no adverse reactions occurred during treatment.Conclusion:Moxibustion combined with small intestine decompression tube can accelerate the recovery of gastrointestinal function,alleviate clinical accompanying symptoms,shorten hospitalization time,improve gastrointestinal hormone levels,and balance the permeability of intestinal mucosal barrier in the treatment of right colon cancer with intestinal obstruction;To a certain extent,it reduces the pain of patients,reduces complications for surgical treatment,and provides adequate intestinal preparation during the perioperative period.Patients do not experience any adverse reactions,and its safety is high,which can be promoted and used in clinical practice. |
