| OBJECTIVE: To provide a more effective method for acupuncture treatment of vertebral artery-type cervical spondylosis by observing the clinical efficacy of acupuncture at the Five-shu Points under the guidance of the Ren Ying-Qi Kou Pulse diagnostic methods.METHODS: Sixty patients with vertebral artery-type cervical spondylosis who met the criteria were randomly divided into two groups: 30 cases in the control group were acupunctured with conventional acupoints,and 30 cases in the pulse method group were acupunctured with the five-shu points(under the guidance of the Ren Ying-Qi Kou pulse method),all treated once/day and 6 times/week for a course of treatment,with an interval of1 day for each course of treatment,for a total of 4 weeks.The ESCV score,TCM vertigo syndrome score and TCD detection index before and after treatment were used as the observation basis,and the efficacy was evaluated at the end of treatment.The ESCV scores and long-term efficacy were evaluated at follow-up 1 month after the end of treatment,and data analysis was performed by SPSS26.0.Results:1.before treatment: comparing the general data(gender,disease duration,age,evidence type),ESCV score(total score,symptom score),TCM vertigo syndrome score(total score,item score),and TCD detection indexes(BA,LVA,Vm of RVA)between the two groups,there was no significant difference by statistical analysis(P>0.05),which could be further compared.2.After treatment:(1)Comparison of ESCV scoresThe comparison of the respective ESCV scores(total score and symptom score)between the two groups before and after treatment showed significant differences(P < 0.05),indicating that both the Ren Ying-Qi Kou pulse method-guided acupuncture method and the conventional acupuncture method could improve the clinical symptoms of patients with vertebral artery-type cervical spondylosis.The ESCV scores(total score and symptom score)were compared between the two groups after treatment in terms of total score and four symptoms("vertigo," "shoulder and neck pain," "daily life and work," "psychological and social adjustment")compared with the control group(P<0.05),indicating that the pulse method group had better improvement in the upper 4 symptoms;while there was no significant difference in the "headache" symptoms(P =0.943>0.05),indicating that the two groups had better improvement in the "headache" symptoms.There was no significant difference in the improvement of "headache" symptoms between the two groups.(2)Comparison of TCM vertigo syndrome scoresWhen comparing the TCM vertigo syndrome points(total score and points of each item)between the two groups before and after treatment,there was a significant difference(P < 0.05),indicating that both groups could improve the TCM syndrome of patients with vertebral artery type cervical spondylosis;when comparing the TCM vertigo syndrome points(total score and points of each item)between the two groups after treatment,there was a significant difference(P < 0.05),indicating that the pulse method group had better improvement of the TCM syndrome.(3)Comparison of TCD detection indexesThe comparison of the respective TCD detection indexes(Vm of BA,LVA and RVA)between the two groups before and after treatment showed significant differences(all P < 0.05),indicating that both groups were able to improve the blood flow of VBA in patients with vertebral artery cervical spondylosis;the comparison of the TCD detection indexes(Vm of BA,LVA and RVA)between the two groups after treatment showed significant differences(P < 0.05),indicating that the pulse method group was more effective in improving the The improvement of blood flow of VBA was better.(4)Comparison of therapeutic efficacyAfter 4 weeks of treatment,the total effective rate of the pulse method group was 90.0%,compared with 76.67% in the control group,and the pulse method group was significantly higher than the control group by statistical analysis(P < 0.05).This indicates that the clinical efficacy of acupuncture of the five infusion points using the Ren Ying-Qi Koupulse diagnostic method is superior to that of acupuncture of conventional acupoints in the treatment of vertebral artery-type cervical spondylosis.3.Long-term effectsAfter 1 month after the end of treatment,all patients obtained follow-up.(1)Comparison of ESCV scoresWhen comparing the respective ESCV scores(total score and symptom score)of the two groups at follow-up and after treatment,there was no significant difference in each item and total score of the control group(P>0.05);the total ESCV score of the pulse method group,"vertigo","neck and shoulder pain",and In the pulse method group,there were significant differences in ESCV total score,"dizziness","neck and shoulder pain" and "psychological and social adaptation"(P < 0.05);there were no significant differences in "headache" and "daily life and work"(P > 0.05).This indicates that from the perspective of the long-term treatment of vertebral artery cervical spondylosis,the conventional acupuncture method did not significantly improve all the symptoms and the overall symptoms;the acupuncture method guided by the Ren Ying-Qi Kou pulse method could improve the overall symptoms,"dizziness","neck and shoulder pain",and "psychological and social adaptation".Psychological and social adjustment",but not "headache" and "daily life and work".(2)Long-term efficacy comparisonThe total effective rate of the pulse method group was 93.33%,and the total effective rate of the control group was 76.67%,which was significantly higher than that of the control group by statistical analysis(P < 0.05).This indicates that the long-term efficacy of acupuncture at the five transfusion points using the Ren Ying-Qi Kou pulse method is superior to that of acupuncture at conventional acupoints in the treatment of vertebral artery-type cervical spondylosis.Conclusion:In the treatment of vertebral artery-type cervical spondylosis,the use of the Ren Ying-Qi Kou pulse method to guide acupuncture at the Five-shu points can better relieve clinical symptoms and improve the therapeutic effect than conventional acupuncture therapy.This study provides a new treatment idea for the treatment of vertebral artery cervical spondylosis with acupuncture,and the Ren Ying-Qi Kou pulse method adheres to the treatment principle of "discerning the pulse and applying acupuncture" in the Nei Jing,which is practical and effective in acupuncture clinics. |
