Objective:By observing the clinical effects and the effect on the skin barrier function of the oral and external Jianpi Runzao Decoction in association with licorice oil to cure atopic dermatitis(spleen deficiency and blood dryness),the mechanism of action and safety of this combination were further investigated in order to provide new ideas for the treatment of atopic dermatitis in Chinese medicine. Methods: A total of 60 patients, all from the Department of Dermatology of the Affiliated Hospital of Jiangxi University of Chinese Medicine, were included in this study, the included patients were randomly divided into treatment and control groups, with 30 cases in each group. The observation group received oral Jianpi Runzao Decoction along with topical licorice oil and the control group taking oral loratadine combined with topical urea cream. The control group took oral loratadine with topical urea cream. The course of treatment is 4 weeks in total. The effectiveness of the curing depend on the variations in lesion area, lesion severity, itchiness,SCORAD total score, skin barrier function test index and symptom score.Side effects and phenomena occurring during treatment have been monitored and the safety of the treatment method has been studied. After 1 month of treatment,follow-up was performed to determine the recurrence rate.Results:1.Baseline comparison:Comparing the data of age, gender, disease duration, total SCORAD score and its various skin lesion subscores, skin barrier function test index and TCM symptom score between the two groups, there were none of the statistical differences(P>0.05),which were comparable;2.Clinical achievement comparison:The effective rate of the control group and treatment group were,respectively,40.0% and 73.3%,and the treatment group’s cure rate was greater than the control group’s(P<0.05);3.Comparison of skin lesion scores: After 4 weeks of treatment, compared with the pre-treatment period, the total SCORAD score and each lesion sub-score in both groups showed a decreasing trend(P<0.05),and the treatment group was better than the control group(P<0.05);4.Comparison of skin barrier function indexes: Throughout 4 weeks of therapy, skin barrier function was enhanced in both groups, and the differences were statistically significant(P < 0.05).Following therapy, there was a higher improvement in the treatment group than in the control group, and this difference was statistically significant(P <0.05);5.Comparison of TCM symptom score: After 4 weeks of treatment, the total TCM symptom score of two groups descend comparing with those prior to treatment(P<0.05),and the treatment group was to fall below the control group(P<0.05);6.Security evaluation: Both groups did not experience any negative side effects throughout therapy, proving the safety of both treatment approaches;7.Comparison of recurrence rate: After a 4-week treatment interruption, the treatment group’s rate of recidivism was 12% as opposed to the control group’s 44.4%.The treatment group’s recidivism rate was significantly lower than that of the control group, and this difference was statistically significant(P<0.05).Conclusion:1.The combination of Jianpi Runzao Decoction with licorice oil was more effective in the treatment of mild to moderate atopic dermatitis of spleen deficiency and blood dryness, effectively reducing lesion area, lesion severity, pruritus, overall SCORAD score and TCM symptom score,as well as repairing the skin barrier.2.Good security and low recurrence rate for mild-to-moderate atopic dermatitis with the combination of Jianpi Runzao Decoction with licorice oil.
