| PurposeBased on the theory of neuroplasticity and the previous research results of the research group,from the perspective of physiology-psychology-society,this study applies the comprehensive intervention mode of cognitive reinforcement to carry out intervention for patients with spinal cord injury,track the rehabilitation effect and investigate their training compliance,and verify the feasibility and effectiveness of this intervention program.To provide clinical reference for improving cognitive function and delaying cognitive impairment in patients with spinal cord injury.MethodThe first step is to select 105 patients with spinal cord injury from two Grade A hospitals in Hefei City from May to October 2021 to carry out comprehensive cognitive strengthening training and conduct questionnaire survey to investigate their training compliance level during rehabilitation and evaluate the feasibility of cognitive strengthening intervention,laying a foundation for clinical control study.The second step was to select 48 patients with spinal cord injury from November 2021 to February 2022 in a Grade A hospital in Hefei as the control group,and adopt routine rehabilitation nursing;A total of 40 patients treated from March to September 2022 were selected as the intervention group,and the comprehensive intervention mode of cognitive reinforcement was implemented on the basis of the control group.Relevant data of patients were collected before,1 month after and 3 months after the intervention.Repeated measure analysis of variance was used to compare the two groups of patients with various indicators to evaluate the effectiveness of the intervention program.ResultThe results of compliance survey showed that the cognitive training compliance of patients with spinal cord injury after staying at home was general,which was mainly affected by the level of physical exercise self-efficacy,objective support and self-care ability of patients.The results of the clinical trial stage showed that the cognitive function score,visual-spatial executive ability score,attentional dimension score,abstract dimension score,memory dimension score,orientation dimension score,social support level score,subjective support dimension score,coping self-efficacy score and anxiety level score were interactive between the two groups of patients(P < 0.05).Simple effect analysis showed that the scores of cognitive function,visuospatial executive ability,attentional dimension,abstract dimension and orientation dimension were improved one month after intervention in the intervention group(P < 0.05),and the scores of memory dimension were significantly changed three months after intervention(P < 0.05).Independent effects between groups showed that the scores of cognitive function,attention dimension and orientation dimension in the intervention group were higher than those in the control group one month after intervention(P <0.05).After 3 months of intervention,the scores of visual spatial executive ability,abstract dimension and memory dimension were significantly higher than those of control group at the same period(P < 0.05).ConclusionThere are certain objective requirements for patients’ own conditions to carry out cognitive intensive training.Before carrying out large-scale clinical application,a variety of influencing factors should be considered.It is necessary to adjust intervention methods and doses appropriately according to patients’ individual conditions.The results of this intervention showed that the comprehensive intervention mode of cognitive enhancement of spinal cord injury can improve the cognitive function of patients with spinal cord injury.Comprehensive interventions can not only delay the process of cognitive impairment,but also improve the psychosocial environment of patients to a certain extent and promote physical and mental rehabilitation of patients. |
