Objective:Flat foot is one of the common foot deformities in clinical practice,which is characterized by low or missing arches.In recent years,the incidence of flat feet in college students is increasing,the long-term development of flat feet will lead to pain and wrong movement patterns,and progress to stiff flat feet,which affects the quality of life of college students.Early intervention is very important for asymptomatic flat feet.By analyzing the changes in morphology and planar pressure of flat foot college students before and after intervention,this paper intends to understand the improvement of flat foot college students by 3D printing insole and exercise intervention,analyze the intervention effect of combined intervention on flat foot,provide guidance for clinical intervention of flat foot by 3D printing insole combined with exercise training,and lay a foundation for the promotion of 3D printing insole.Methods:College students in our school were screened for flat feet,and the differences of foot characteristics between normal feet and flat feet were analyzed by foot pose index score FPI-6,navichoid drop test NDT,arch index AI and plantar pressure characteristics.A total of 152 students with flat feet were recruited and divided into four groups by random number table method: blank control group(F0),3D printed insole group(F1),sports training group(F2)and 3D printed insole group combined with sports training group(F3).F0 did not take intervention measures,F1 wore 3D printed insole and F2 took exercise training.F3 was 3D printed insole combined with exercise training,which lasted for a total of 8 weeks.The data of plantar pressure,foot posture index score,arch index and scaphoid fall test of all patients at week 0 and week 8 were collected by plantar pressure system,2D scanning system and related rating table,and statistical analysis was conducted by SPSS26.0 to determine the effect of different intervention measures on flat foot college students.Results:1.The morphology and plantar pressure characteristics of flat foot students are different from those of normal foot students.FPI-6>6 in flat feet;NDT greater than10mm;Arch index is generally greater than 28%.The contact area of flat feet was significantly increased in the M1 and MF regions(P<0.05).The stress and pressure peaks in M1,MF,MH and LH were significantly increased compared with those in normal feet(P<0.05).2.Morphological changes after intervention,FPI-6 and NDT in 3D insole group were significantly changed(P<0.05);There were significant changes in FPI-6 in exercise group(P<0.05),and very significant changes in NDT and AI compared with before intervention(P<0.01),and very significant changes in FPI-6,NDT and AI in combination group(P<0.01).3.Static plantar pressure results showed that the contact area of MF region in the insole group was significantly decreased(P<0.05),the stress and contact area of MF and the peak pressure and force of M1 in the exercise group were significantly decreased(P<0.05),and the plantar pressure in the M1,MF and MH region in the combined group was significantly decreased compared with before intervention(P<0.05).4.The results of dynamic plantar pressure showed that the stress in the MF region decreased significantly in the insole group(P<0.05),the peak pressure in the MH region and the stress in the MF region decreased significantly in the exercise group(P<0.05),and the plantar pressure index in the MF and MH region changed significantly in the combined group compared with before intervention(P<0.05).Conclusion:1.There are different characteristics of plantar pressure in the feet of flat feet and normal feet.2.3D printing insole,exercise training and combined intervention are effective in improving flat feet.3D printing insole can adjust the distribution of plantar pressure by supporting the medial longitudinal arch and related modules;3.Sports training can enhance the strength of muscles and ligaments supporting the medial longitudinal arch and reduce plantar injury and structural deformation;4.The combined intervention with exercise and 3D printing insole has better effects on improving the morphology of flat feet and adjusting the abnormal planar pressure.
