| Purpose:To explore the long-term blood pressure control rate of the elderly in Baishishan Town,Jiaohe City,Jilin Province and its influencing factors to provide evidence-based medical reference for the control and management of hypertension in the elderly in Baishishan Town,Jiaohe City.Method:In Baishishan Town,Jiaohe City,elderly hypertensive patients who participated in free physical examination for three consecutive years from 2019 to 2021 were selected as the research object,and all elderly hypertensive cases in 2019,a total of979 cases,were followed up until 2021.All subjects have complete clinical data,including blood pressure examination records,blood pressure grade registration,general demographic data,living habits data and examination data(liver function,renal function,blood lipid,fasting blood glucose,height,weight and waist circumference).After the physical examination in 2021,investigate the treatment of hypertension in each patient,ask about the knowledge of hypertension,and evaluate the standard of blood pressure control.According to the blood pressure classification,all patients were divided into three subgroups: grade 1,grade 2 and grade 3,and were divided into standard group and substandard group according to the blood pressure control in 2021.First of all,observe the compliance rate of blood pressure control in elderly hypertensive patients in Baishishan Town,Jiaohe City;Secondly,the general demographic data,living habits data,routine examination data,knowledge of hypertension and blood pressure control methods of grade 1,grade 2 and grade 3hypertension groups and unqualified groups were compared respectively,and univariate analysis was carried out.Finally,on the basis of single factor analysis,multivariate binary Logistic regression analysis was carried out to determine the blood pressure compliance of patients with different grades of hypertension and its influencing factors.Results:1.Among the 979 subjects,117 reached the standard and 862 failed to reach the standard,with a rate of 14.29%.The compliance rate of Grade 1 hypertension patients was 15.09%,grade 2 hypertension was 13.76% and grade 3 hypertension was 6.74%.The majority of grade 1 to 3 patients who failed to reach the standard were SBP,and the proportion of patients who failed to reach the standard of DBP,SBP and DBP were low.The higher the hypertension grade,the lower the blood pressure meeting the standard(P<0.05);With the aggravation of hypertension,the proportion of SBP not reaching the standard was higher(P<0.05).2.The proportion of elderly patients with grade 1 hypertension combined with diabetes and hyperlipidemia was higher than that in the standard group(P<0.05);The proportion of patients with non-standard FBG,abdominal obesity and overweight/obese BMI in non-standard group was higher than that in the standard group(P<0.05).The proportion of patients who knew more about hypertension,salt restriction,diet control,exercise adherence,smoking cessation,single drug and combined drug treatment in the non-standard group was lower than that in the standard group(P<0.05).Binary Logistic regression analysis showed that FBG sub-standard,abdominal obesity and non-medication were the risk factors for non-standard blood pressure control in elderly grade 1 hypertension patients(OR>1,P<0.05).Better knowledge of hypertension,salt restriction,exercise persistence and monotherapy were the promoting factors for blood pressure control in the elderly patients with grade 1 hypertension(OR<1,P<0.05).3.The proportion of elderly patients living alone in the non-conforming group was higher than that in the conforming group(P<0.05);The proportion of patients with diabetes mellitus,hyperlipidemia and cerebral infarction in non-standard group was higher than that in standard group(P<0.05).The proportion of patients with elevated TC,substandard FBG,abdominal obesity and overweight/obese BMI in the non-standard group was higher than that in the standard group(P<0.05).The proportion of patients who knew more about hypertension,salt restriction,diet control,exercise adherence,smoking cessation,single drug and combined drug treatment in the non-standard group was lower than that in the standard group(P<0.05).Binary Logistic regression analysis showed that FBG sub-standard,abdominal obesity and no medication were the risk factors for non-standard blood pressure control in elderly patients with grade 2 hypertension(OR>1,P<0.05).Better understanding of hypertension knowledge,salt restriction,exercise persistence,dual drug therapy and three-drug combination therapy were the promoting factors for blood pressure control in the elderly patients with grade 2 hypertension in the town(OR<1,P<0.05).4.The proportion of elderly patients aged ≥80 years old and living alone in the non-standard group of Grade 3 hypertension was higher than that in the standard group(P<0.05);The proportion of patients with diabetes,hyperlipidemia,and coronary heart disease in non-standard group was higher than that in standard group(P<0.05).The proportion of patients with non-standard FBG,abdominal obesity and overweight/obese BMI in non-standard group was higher than that in the standard group(P<0.05).The proportion of patients who knew more about hypertension,salt restriction,diet control,exercise adherence,smoking cessation,single drug and combination drug treatment in non-standard group was lower than that in standard group(P<0.05).Binary Logistic regression analysis showed that age ≥80 years old,FBG sub-standard,combined with hyperlipidemia,cerebral infarction,abdominal obesity,BMI overweight/obese,and no medication were the risk factors for non-standard blood pressure control in elderly grade 3 hypertension patients in the town(OR>1,P<0.05).Better understanding of hypertension knowledge,adherence to exercise,dual drug therapy and three-drug combination therapy were the promoting factors for blood pressure control in the elderly patients with grade 3 hypertension(OR<1,P<0.05).Conclusion:1.The compliance rate of hypertension in the elderly in Baishishan Town,Jiaohe City,Jilin Province is low,and SBP is the main hypertension at all levels,and the higher the hypertension grade,the lower the compliance rate.2.Failure to meet the standard of FBG,abdominal obesity and no medication are the risk factors for the failure to meet the standard of hypertension control in grade 1and grade 2 of the elderly in this town,and better understanding of hypertension knowledge,salt restriction,persistent exercise,single drug and combined drug treatment are the promoting factors for the failure to meet the standard of hypertension control in grade 1 and grade 2 of the elderly in this town.3.Age ≥ 80 years old,FBG failing to meet the standard,hyperlipidemia,abdominal obesity,overweight/obesity of BMI,and no medication are the risk factors for failing to meet the standard of blood pressure control of elderly patients with grade3 hypertension in this town,and better knowledge of hypertension,persistent exercise and combined medication are the promoting factors for reaching the standard of blood pressure control of elderly patients with grade 3 hypertension in this town. |
