ObjectiveTo explore the effects of mindfulness-based cognitive therapy on anxiety, sleep quality, pregnancy outcome and mindfulness-based level of patients undergoing in vitro fertilization-embryo transfer.Methods1.Construct the intervention program: Based on the theoretical framework of the three-axis model of mindfulness and the two-level model of cognitive(primary)and meta-cognitive(secondary)arousal,the Eight-week Mindfulness Journey from Depression and Emotional Stress translated by John Tisdale. The first draft of the intervention plan was formulated according to the actual situation of IVF-ET patients and the specificity of the treatment process. The first draft of the intervention plan was discussed and revised by the expert meeting method, and the application draft of the intervention plan was determined on the basis of the pre-experiment.2.Implementation of intervention: a questionnaire survey was conducted among female patients undergoing IVF-ET in the Reproductive Medicine Department of the First Affiliated Hospital of Dali University from January 1,2022 to October 1,2022.First,the patients’ wishes were solicited, the research objects were numbered, and they were randomly divided into the control group and the intervention group according to the patients’ wishes. The intervention group was compared at eight time points of the in vitro fertilization-embryo transfer treatment cycle(the day of down-regulation,7 days after down-regulation,14 days after down-regulation,21 days after down-regulation)Day,the day of ovulation induction, the day of men’s ejaculation, the day of needle change, and the day of embryo transfer)plus a total of 8 times(7 times of online mindfulness-based cognitive intervention in Tencent conference group and 1 time of offline one-on-one mindfulness-based cognitive intervention).Patients in the control group were given routine care. Before the intervention on the day of down regulation(T1 time point),the women registered to fill in the IVF-ET patients’ general information questionnaire,SAS,PSQI and FFMQ.SAS,PSQI and FFMQ were filled out again on the day of embryo transfer(T2 time point)and 14 days after embryo transfer(T3 time point). On the 14 th day after embryo transfer, the patient’s peripheral venous blood was collected to detect β-hc G to determine the pregnancy outcome. The differences of SAS,PSQI,FFMQ,biochemical pregnancy rate and clinical pregnancy rate between the two groups were analyzed.Results1.The basic information of the subjects included the basic demographic information of the intervention group and the control group, mainly including age, ethnicity, education level, current status and family percapita monthly income and other related variables(P >0.05).2.Before intervention(T1 time point),there were no significant differences in SAS,PSQI,FFMQ scores and total scores between the two groups(P>0.05).3.After the intervention, there were significant differences in SAS scores between the two groups on the day of embryo transfer(T2 time point)and 14 days after embryo transfer(T3 time point)(P<0.05).4.After the intervention, there were no significant differences in the scores of sleep quality, sleep latency, sleep efficiency and sleep disorder in PSQI scale between the two groups(P 均> 0.05).However,there were significant differences in sleep time, daytime dysfunction and PSQI total score(P<0.05).5.After the intervention,it was observed that there was no significant difference in the scores of non-judgment and non-reaction of FFMQ scale between the two groups(P 均>0.05).However,there were significant differences in observation, description, aware action and FFMQ total score(P<0.05).6.There was no significant difference in the distribution of biochemical pregnancy and clinical pregnancy between the two groups(P > 0.05).Conclusion1.Mindfulness-based cognitive intervention can alleviate the anxiety of patients undergoing in vitro fertilization and embryo transfer, and improve the level of mindfulness awareness;2.Mindfulness-based cognitive intervention has no significant effect on improving sleep quality, biochemical pregnancy rate and clinical pregnancy rate. However, the clinical application effect is obvious, which can be used as an important nursing method to improve the pregnancy outcome of patients with in vitro fertilization-embryo transfer.
