ObjectiveBy measuring the concentrations of ACE2 and Ang(1-7)in serum and follicular fluid of patients with PCOS undergoing IVF-ET and patients in the control group(tubal factor or male factor infertility)were measured,statistical analysis was conducted to compare the differential expression of ACE2 and Ang(1-7)in serum and follicular fluid of patients with PCOS and patients in the control group,and analyzing the relationship between the expression of them and oocyte maturity,To analyze the risk factors of PCOS and the influencing factors of oocyte maturation,and to explore the role of ACE2 and Ang(1-7)in PCOS and its influence on oocyte maturity.MethodsAccording to the inclusion criteria and exclusion criteria,the PCOS group and the control group were enrolled.The patients’ age,marital status,menstrual status,BMI,family history,ovulation induction protocol,total Gn amount,number of oocytes retrieved,number of mature oocytes and other clinical data were collected.The basal serum concentrations of FSH,LH and AMH were measured by electrochemiluminescence immunoassay.The experiment was divided into two parts.In the first part,10 PCOS patients and 21 controls were included.In the second part,11 patients with PCOS and 21 controls were included.The serum ACE2 and Ang(1-7)concentrations were measured by ELISA before the cycle.SPSS27.0 statistical software was used to analyze the data.The measurement data were expressed as(χ±s)or quartile,and the comparison between the two groups was performed by t test or non-parametric test.Pearson correlation analysis was used to analyze the factors related to oocyte maturity.Binary regression analysis was used to analyze the independent risk factors of PCOS and oocyte maturity,and P<0.05 was considered statistically significant.Results1.The basal serum levels of LH,AMH and LH/FSH in PCOS patients were higher than those in the control group(P<0.05),When there was no significant difference in the total dosage of Gn and BMI between the two groups,the number of oocytes retrieved and mature oocytes in PCOS patients were higher than those in the control group(P<0.05),but the maturity of oocytes was lower than that in the control group.2.The concentrations of ACE2 and Ang(1-7)in peripheral blood and follicular fluid of PCOS patients were lower than those of control group(P<0.05).3.Oocyte maturity was positively correlated with ACE2 and Ang(1-7)contents in follicular fluid,and negatively correlated with serum LH concentration and LH/FSH.4.There was no significant correlation between serum ACE2 and Ang(1-7)levels and oocyte maturity,In the age range of 24-35 years old,serum ACE2 and Ang(1-7)levels were positively correlated with age,and negatively correlated with serum LH concentration and LH/FSH.Conclusions: 1.PCOS patients aged 24-35 years have better fertility potential than those with tubal infertility of the same age.2.PCOS patients have high LH level,high level of AMH and other neuroendocrine disorders;3.RAS system exists in human ovary,and there is no high expression of ACE2 and Ang(1-7)in PCOS patients.4,ACE2 and Ang(1-7)may be closely related to oocyte maturation,ACE2 and Ang(1-7)may be mediated by high LH.To a certain extent,local high levels of ACE2 and Ang(1-7)may promote oocyte maturation.
