Objectives:Under the background of aging society and the trend of increasing prevalence of type 2 diabetes combined with osteoporosis year by year,based on the research conclusion that vitamin D is related to both type 2 diabetes and osteoporosis,combined with the characteristics of multiple diseases co-existing in the elderly population.To study the influencing factors of vitamin D deficiency in elderly patients with type 2 diabetes mellitus complicated with osteoporosis,construct the overall concept of disease treatment,and provide a new idea for clinical prevention and treatment of type 2 diabetes mellitus complicated with osteoporosis in the elderly population.Methods:A cross-sectional study was conducted to collect patients aged 60 years and above who were admitted to the Department of Endocrinology of the First People’s Hospital of Yunnan Province from January 2020 to December 2020 and diagnosed with type 2 diabetes mellitus combined with osteoporosis.General data of patients were collected,including age,height,weight,body mass index [body weight(kg)/height(m)square]kg/m2,diabetes course;Laboratory indicators include fasting blood glucose(FBG),glycated hemoglobin(Hb A1c),hemoglobin(HGB),albumin(ALB),triglycerides(TG),cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),uric acid(UA),estradiol(E2),testosterone(T),triiodothyronine(T3),free triiodothyronine(FT3),thyroxin(T4),free thyroxin(FT4),thyroid stimulating hormone(TSH),25 hydroxyvitamin D(25(OH)VitD),blood calcium(Ca).According to the serum 25(OH)VitD level and the clinical diagnostic criteria for vitamin D deficiency formulated by the American Endocrine Association in 2011 [1],vitamin D level was divided into the following groups: Vitamin D deficiency group(Group A): 25(OH)VitD ng/m L;Vitamin D deficiency group(Group B): 25(OH)VitD was 20.0-29.9 ng/m L;Vitamin D deficiency group(C group): 25(OH)VitD ≥30.0ng/m L.Results:1.Univariate analysis showed that patients in the vitamin D deficiency group,the vitamin D deficiency group,and the vitamin D adequate group There were significant differences in body mass index,fasting blood glucose,glycosylated hemoglobin,hemoglobin,albumin,triglyceride,high density lipoprotein cholesterol,uric acid,estradiol,testosterone,triiodothyronine and blood calcium(P<0.05).2.Spearman correlation analysis showed that fasting blood glucose,glycosylated hemoglobin and 25(OH)VitD levels were negatively correlated(P<0.05).Hemoglobin,albumin,estradiol,testosterone,triiodothyronine and blood calcium were positively correlated with 25(OH)VitD levels(P<0.05).Age,diabetes course,body mass index,triglyceride,cholesterol,HDL cholesterol,LDL cholesterol,uric acid,thyrotropin,thyroxine,and free thyroxine were not correlated with 25(OH)VitD levels.3.Multivariate Logistic regression analysis showed that albumin(OR =0.84,P<0.05)and testosterone(OR =0.944,P<0.05)were independent influencing factors of vitamin D deficiency in elderly type 2 patients with osteoporosis.Conclusions:1.In elderly patients with type 2 diabetes and osteoporosis,The levels of BMI,FBG,Hb A1 c,HGB,ALB,TG,HDL-C,UA,E2,T,FT3 and Ca were different in patients with different levels of 25(OH)VitD.2.The level of 25(OH)VitD in elderly patients with type 2 diabetes mellitus and osteoporosis is associated with multiple factors.3.Albumin and testosterone are protective factors for vitamin D deficiency. |
