Objective: To observe the clinical effect of spinal manipulation in the treatment of type 2diabetes mellitus with Yin deficiency and heat excess, and to provide clinical basis for spinal manipulation in the treatment of type 2 diabetes mellitus with Yin deficiency and heat excess. Methods: A total of 72 patients who met the inclusion criteria were recruited from the outpatient and(treatment)area of Endocrinology department and Tuina Department in the First and Third Clinical hospitals Affiliated to Changchun University of Traditional Chinese Medicine. They were randomly divided into control group and experimental group, with 36 patients in each group. The experimental group was treated with basic treatment +metformin hydrochloride tablets + spinal manipulation. Each patient was treated for 2courses,2 weeks as a course. Each patient was individually quantified for symptoms before and after treatment, and the traditional Chinese medicine syndrome scale of diabetes and glucose metabolism index were used as the evaluation criteria for therapeutic effect. Results: A total of 72 patients were enrolled in this clinical protocol,of which 3 cases dropped out,2 cases were excluded, and 67 cases were effective.1.Evaluation of clinical efficacy: after treatment,in the control group,13 cases were markedly effective,12 cases were effective,8 cases were ineffective, and the total effective rate was 73.53%;In the experimental group,20 cases were markedly effective,13 cases were effective, and 1 case was ineffective, with a total effective rate of 97.06%.2.Evaluation of TCM syndrome score: after treatment, the total TCM syndrome scores of the two groups were significantly lower than those before treatment, and the difference was statistically significant(P < 0.05),and the curative effect of the experimental group was better than that of the control group.3.Evaluation of glucose metabolism indexes: after treatment, the clinical indexes of FBG,2h BG and Hb A1 c of the two groups were significantly improved compared with those before treatment, and the differences were statistically significant(P < 0.05).Among them, the improvement of glucose metabolism indexes in the experimental group has certain advantages compared with the improvement of glucose metabolism indexes in the control group. Conclusion: Spinal manipulation in the treatment of type 2 diabetes with Yin deficiency and heat excess can effectively reduce the glucose metabolism index, and the overall curative effect is better than that of the control group. No adverse events such as damage to heart, brain, liver and kidney function are found during the treatment, which is safe and effective.Spinal manipulation can effectively improve the related symptoms of Yin deficiency and heat excess syndrome of type 2 diabetes mellitus, and can effectively improve the patients’ TCM syndrome score.Spinal manipulation is effective in the treatment of type 2 diabetes with Yin deficiency and heat excess, which can provide clinical reference for the treatment of type 2 diabetes.
