The Analysis Based On The TCM Constitution Correlation Survey Of Patients With Thyroid Nodule In Our Hospital

Objective: To summarize the correlation between the formation of thyroid nodule and the constitution distribution of traditional Chinese medicine,and to explore the distribution and regularity of the main constitution types of thyroid nodule patients in our hospital,which has important reference value for the prevention and treatment of thyroid nodule.Methods: A total of 215 patients with thyroid nodule who visited the outpatient department and treatment area of Endocrinology and Metabolism Disease,Affiliated Hospital of Changchun University of Traditional Chinese Medicine from January 2022 to December 2022 were selected as the research objects.After signing the informed consent form,the patients were tested for physique typing by TCM physique score scale,and ultrasound and other examination data were collected.Finally,Excel and SPSS22.0statistical software were used for analysis to observe the relationship between physical fitness and thyroid nodule.Results: A total of 215 patients were observed in this study,which were divided into nine types of physique for study.Among them,patients with Qi stagnation type were the most,accounting for 16.3%,while patients with peace type and characteristic type were less,accounting for 5.6% and 1.4% respectively.In terms of gender,male patients accounted for43.7% and female patients accounted for 56.3%,among which,phlegm-dampness and air-depression patients accounted for more female patients,63.3% and 68.6%,respectively.In terms of age,the incidence of thyroid nodules increased with age,and the proportion of young,middle-aged and elderly patients was 17.7%,37.2% and 45.1%,respectively.There was no significant difference in the age distribution of different TCM constitution.In terms of BMI,overweight patients accounted for the highest proportion of 42.3%,among which Yang deficiency and phlegm-dampness overweight and obese patients accounted for a relatively high proportion of 81.8% and 83.3%,respectively.In terms of personal history and family history,Yin deficiency quality and phlegm-dampness quality accounted for more in patients with smoking history,accounting for 75.0% and 83.3%;blood stasis quality accounted for more in patients with family genetic history,accounting for 71.4%;drinking history had no significant influence on the prevalence rate of thyroid nodules.In terms of ultrasound examination,in the grading of TIRADS,the nodules of Yin deficiency and Yang deficiency were more in grade 4c and Grade 5,accounting for 57.1% and 68.2%.In the boundary manifestations,there were more borderless nodules of Yang deficiency and Yin deficiency,accounting for 63.6% and 67.9%.Patients with Yin deficiency,Yang deficiency and blood stasis had more irregular nodules,accounting for 60.7%,68.2% and 71.4%.The CDFI of thyroid nodules with Yang deficiency showed blood flow signal in more patients(77.3%).The abnormal antibody of Yin deficiency substance,phlegm dampness substance and qi stagnation substance accounted for 67.9%,70.0% and 82.9%,respectively,suggesting Hashimoto’s thyroiditis.Conclusion:1.Qi-stagnation is most common in thyroid nodule patients;In terms of gender,female patients were more likely to suffer from this disease,and phlegm-dampness and air-stagnation accounted for more in female patients.The incidence of thyroid nodule increased with age.In terms of BMI,overweight patients accounted for the highest proportion,and Yang deficiency and phlegm-dampness overweight and obese patients accounted for a higher proportion.Yin deficiency and phlegm-dampness accounted for more in patients with smoking history.Family genetic history suggests that blood stasis is more susceptible to disease.Therefore,patients with the above related factors should pay attention to the prevention of thyroid nodules.2.Among patients with thyroid nodules,patients with constitutions of Yang deficiency,Yin deficiency,and blood stasis have poor prognosis in terms of ultrasonic TIRADS grading,boundary manifestations,morphological status,and whether there is blood flow signal.Therefore,patients with thyroid nodules of these three constitutions should pay attention to adjusting their living habits and conducting regular review to avoid nodules deterioration.
