Objective:To evaluate the clinical efficacy and practicality of the combination of Professor Ren Jixue’s foot soaking and acupoint pasting therapy in treating CKD3 with hypertension(spleen-kidney yang deficiency pattern),and to verify the external treatment method for treating patients with CKD3 complicated with hypertension(spleen-kidney yang deficiency pattern)is an effective and convenient antihypertensive therapy. Methods: This clinical research project collected 72 patients from the Department of Nephrology at JILIN Province Hospital Of TCM who met the inclusion criteria of this study from December 2021 to December 2022 through a prospective randomized controlled trial. The patients were randomly divided into a control group(n=36)and a treatment group(n=36).Patients in the control group received symptomatic treatment with western medicine and levamlodipine besylate tablets(Shi huida pharmaceutical group(jilin)co.LTD,National Medicine Approval Number H19991083)at a dose of 2.5mg to 5mg.The treatment group received the same treatment as the control group plus Professor Ren Jixue’s foot soaking and acupoint pasting therapy. The treatment period for both groups was 4 weeks. The antihypertensive effect,TCM syndrome score, antihypertensive drug withdrawal rate, blood creatinine, blood urea nitrogen, and glomerular filtration rate were used as efficacy evaluation indicators. All collected data were analyzed using SPSS 26.0.Results:In this clinical research project,35 patients in the treatment group and 35 patients in the control group completed the treatment, and one patient dropped out from each group. The statistical analysis showed as follows:(1)Effect on TCM syndrome score: The effective rate of TCM syndrome score in the treatment group was 82.9%,which was significantly higher than that in the control group(62.8%)after treatment(P<0.05),indicating that the treatment group was superior to the control group in improving the TCM syndrome score.(2)Effect on clinical symptoms: After treatment, there was a significant improvement in dizziness, cold limbs, soreness and coldness of the waist and knees, poor appetite and distension in the epigastrium, and blurred vision in both groups, with P values <0.05.The results of the analysis showed no significant differences between the two groups in terms of nocturia, fatigue, mental fatigue, edema, and diarrhea(P>0.05).(3)Antihypertensive effect: The total effective rate of antihypertensive treatment in the treatment group was 91.5%,which was significantly higher than that in the control group(74.3%)(P<0.05),indicating that the antihypertensive effect in the treatment group was superior to that in the control group.(4)Effect on blood pressure reduction: After treatment, the comparison of SBP and DBP within the treatment group and control group showed P <0.05,indicating that both groups had antihypertensive effects. The comparison of SBP and DBP between the two groups after treatment also showed P <0.05,indicating that the treatment group was superior to the control group in reducing SBP and DBP.(5)Effect on laboratory and physical examinations: After treatment, both groups showed a significant effect in reducing Scr and BUN,with a statistically significant difference between the two groups(P<0.05),indicating that the treatment group was superior. There was no significant difference between the two groups in terms of improving GFR(P>0.05).(6)Effect on antihypertensive drug withdrawal rate: During the treatment process, the total withdrawal rate of western antihypertensive drugs in the treatment group was 11.4%,and that in the control group was 2.9%.The statistical analysis showed no significant difference between the two groups(P>0.05),indicating that there was no significant difference in the drug withdrawal rate between the two groups.(7)Safety indicators: No adverse reactions occurred during the entire clinical research project, and there was no significant difference between the two groups in terms of liver function, blood routine, and other safety indicators after treatment. Conclusion:This clinical study demonstrates that the combination of western medicine symptomatic treatment, traditional Chinese medicine basic treatment, and external treatment can effectively improve clinical symptoms, reduce SBP,DBP,Scr,and BUN in patients with CKD3 complicated with hypertension(spleen-kidney yang deficiency syndrome). The treatment is safe and reliable, and there were no adverse reactions or significant fluctuations in safety indicators. Therefore,it can be concluded that external treatment is an effective and convenient antihypertensive therapy for patients with CKD3 complicated with hypertension(spleen-kidney yang deficiency syndrome).
