Objective:To explore the auxiliary effect of five-tone nursing on insomnia patients with heart and spleen deficiency treated by abdominal massage in the process of improving sleep quality.Method:1.Subjects and groups: From December 2021 to December 2022,74 cases of insomnia with deficiency of both heart and spleen were selected from the clinic of Massage Department in a 3A hospital in Changchun.Patients were randomly divided into intervention group and control group with 37 cases in each group.2.Intervention Measures The control group was treated with abdominal massage,which was based on the "Diagnosis and Treatment Plan for Insomnia Patients with Deficiency of Heart and Spleen in Massage Department" of a 3A hospital in Changchun.The intervention group was given five-tone nursing on the basis of abdominal massage therapy,including five steps: health education,music selection,music audition,music therapy and asking patients.Intervene five times a week,35 min each time,for 4 weeks,and follow up at the eighth week after treatment.3.Evaluation indicators: General information of patients in the two groups before enrollment SAS,Self-Rating Anxiety Scale(SAS),self-rating depression scale(SDS),Pittsburgh Sleep Quality Index,PSQI),Athens Insomnia Scale(AIS),Fatigue Scale-14(FS-14),sleep log.SAS and SDS were compared only before intervention.PSQI,AIS and FS-14 were evaluated before intervention,4 weeks later and 8 weeks later,and sleep logs were filled out every day during the four-week intervention.4.Statistical methods The counting data were described by frequency and percentage(%),and compared between groups by X2 test.The measurement data were tested for homogeneity of variance,and the repeated measurement analysis of variance was used when the variance was homogeneous and normal.When it is not satisfied,Mann-Whitney-U rank sum test of two independent samples is used for inter-group comparison,and--Friedman test,a nonparametric test of multiple related samples,is used for intra-group comparison.SPSS21.0 was used to analyze the data.Results:1.There was no statistical difference in age,sex,course of disease,occupation,anxiety and depression between the two groups(P>0.05).2.Comparison of PSQI scores Before the intervention,there was no significant difference between the total score of PSQI and the item score between the two groups(P>0.05),but there was significant difference between the four-week and eight-week follow-up periods(P<0.01).There was no significant difference in the scores of six factors(sleep quality,sleep time,sleep efficiency,sleep disorder,sleep duration and daytime dysfunction)of PSQI before intervention,but there was significant difference in the scores of sleep quality,sleep time,sleep efficiency and daytime dysfunction between the two groups during the 4-week and8-week follow-up periods(P<0.05).3.Comparison of AIS scores There was no significant difference in AIS scores between the two groups before intervention(P>0.05),but there was significant difference between the four-week and eight-week follow-up periods(P<0.01).4.Comparison of FS-14 scores There was no significant difference in the total score of FS-14 and the item score between the two groups before the intervention(P>0.05).There was significant difference in the total score of FS-14 between the two groups during the 4-week and8-week follow-up period(P<0.01).There was no significant difference in FS-14-related items between the two groups before intervention(P>0.05),but there was significant difference in the scores of the four-week and eight-week follow-up periods of physical fatigue intervention between the two groups(P<0.05),and there was no significant difference in the scores of the four-week and eight-week follow-up periods of mental fatigue intervention between the two groups(P>0.05).5.After the first week of sleep log comparison intervention,there was no statistical difference in the number of awakenings and the length of sleep between the two groups(P>0.05).After the second week of intervention,there was no statistical difference in the number of awakenings between the two groups(P>0.05),but there was statistical difference in the score of sleep duration(P<0.05).After the third week of intervention,the scores of awakening times and sleep duration between the two groups were statistically significant(P<0.05).After the fourth week of intervention,the scores of arousal times and sleep duration between the two groups were statistically significant(P<0.05).Conclusion:1.Five-tone care plays an auxiliary role in improving the sleep quality,sleep time and sleep efficiency,and improving the daytime dysfunction of patients with insomnia treated by abdominal massage.2.Abdominal massage therapy assisted by five-tone nursing is superior to the control group in improving sleep duration and awakening times,and in improving the severity of insomnia,which has the clinical effect of improving patients’ physical fatigue.
