Systematic Review And Meta-Analysis Of Qingre Huayu Qutan Method In The Treatment Of Acute Ischemic Stroke

Objective:To evaluate the clinical efficacy of Qingre Huayu Qutan method in the treatment of AIS,to explore the advantages of traditional Chinese medicine in the treatment of this disease,and to provide reliable scientific basis for future academic research and clinical treatment.Methods:According to the established retrieval strategy,six databases,such as VIP,CNKI,Wanfang Data Knowledge Service Platform,CBM,Pub Med database and Cochrane Library database,were comprehensively searched to obtain all RCTs on the treatment of AIS Huayu Qutan method from the establishment of the database to January 2022.The literatures were screened according to the inclusion and exclusion criteria,and the data were extracted by Excel software.Rev-Man 5.4 software was used to evaluate the quality of the included literature,and the outcome indicators were statistically analyzed.Results:A total of 93 articles were retrieved and 19 articles were finally included.A total of 2150 RCTs were included.Results of statistical analysis of outcome data:(1)Total clinical effective rate:A total of 12 studies evaluated the total clinical effective rate.A total of 12 studies included 762 patients in the treatment group,692 cases were effective,and the total effective rate was 91%.In the control group,756 cases were effective,581 cases were effective,and the total effective rate was 77%.Heterogeneity test results:x~2=2.86,p=0.99,I~2=0%.Because p value>0.05,the heterogeneity is small.Meta-analysis results:OR[95%CI]=3.03[2.24,4.09],Z=7.22(p<0.00001),the difference was statistically significant,suggesting that the total effective rate of Qingre Huayu Qutan prescription or the prescription combined with western medicine in the treatment of acute ischemic stroke was higher than that of western medicine alone.(2)Neurological deficit:There were 8 studies on the evaluation of neurological deficit.Heterogeneity test results:p<0.00001,I~2=93%,suggesting that there was significant heterogeneity between studies,so the random effect model was selected for Meta analysis.The results showed that:SMD[95%CI]=-1.20[-1.75,-0.66],Z=4.35(p<0.00001),suggesting that the difference between the results was statistically significant,suggesting that Qingre Huayu Qutan Recipe or the prescription combined with conventional western medicine therapy for acute ischemic stroke neurological improvement is better than western medicine alone.(3)Activities of daily living:A total of 9 studies evaluated the ability of daily life.Heterogeneity test results:p<0.00001,I~2=94%,suggesting that there was significant heterogeneity between studies,so the random effect model was selected for Meta analysis.The results showed that:SMD[95%CI]=0.46[-0.15,1.07],Z=1.48(p=0.14),suggesting that the improvement of daily living activities of acute ischemic stroke by Qingre Huayu Qutan Recipe or the combination of Qingre Huayu Qutan Recipe and conventional western medicine was better than that of western medicine alone,but the difference was not statistically significant.Conclusion:(1)Qingre Huayu Qutan treatment of acute ischemic stroke can significantly improve the total clinical efficiency.(2)Qingre Huayu Qutan treatment of acute ischemic stroke can effectively improve neurological deficits,improve the ability of daily life activities.(3)The effect of Qingre Huayu Qutan Recipe or Qingre Huayu Qutan Recipe combined with western medicine is better than that of western medicine alone in the treatment of AIS.
