Purpose: Postoperative pain is the most common complication of patients with mixed hemorrhoids after surgery.It is also the problem that patients are most worried about.Therefore,it has become the main reason that patients have surgical indications but have not performed surgery.Mixed hemorrhoids account for the largest proportion of anal diseases.This clinical study is to verify the simple,safe and effective treatment of mixed hemorrhoids by comparing the treatment of pain after external strip and internal ligature by acupuncture at Chengshan Point and Shugupoint and the treatment of pain after mixed hemorrhoids with oral drugs.Through the study,the clinical effective rate of acupuncture in the treatment of postoperative anal pain was observed,so as to form an application scheme for the clinical application of analgesic methods in the future.Besides,acupuncture in the treatment of postoperative pain of mixed hemorrhoids also has a good effect on various postoperative complications such as anal edema and anal swelling.Methods: According to the randomness and control of the scientific research design,60 patients admitted to the Anorectal Center of the Affiliated Hospital of Changchun University of Chinese Medicine from January 2022 to December 2022 after external stripping and internal ligation of mixed hemorrhoids were selected.These patients should meet our inclusion and exclusion criteria,and were randomly divided into the treatment group of 30patients: When VAS > 3 on the first day after surgery,acupuncture was performed at bilateral Chengshan point and Shugu point.After dressing change in the morning of 2 and 3days after surgery,VAS scores were recorded 1h after dressing change on 1d,2d,3d and 7d.Control group of 30 patients: VAS > 3 hours on the first day after surgery oral administration of acetaminophen hydrocodone tablets,2 and 3 days after surgery in the morning after dressing change,VAS scores were recorded 1 day,2 day,3 day after dressing change 1 hour and 7 day after dressing change 1 hour.The main outcome measures were postoperative pain scores of the two groups.The observation time points were 1,2,and 3days after surgery.VAS pain scores were recorded according to patients’ descriptions according to their pain conditions after interrogation.Secondary outcome measures: wound edema,urinary retention and sleep quality 1,2 and 3 days after surgery.At the same time record The Times and dosage of oral medication,and record the adverse reactions.Results: There were 28 cases in the treatment group,2 cases in the midway shedding,and a total of 30 cases in the control group.Statistical analysis of gender and age showed no statistical significance between the two groups.There were significant differences in pain scores in the treatment group at 3 and 7 days after surgery,and no statistical significance in pain scores in the two groups at 1 and 2 days after treatment(P>0.05),the treatment group was better than the control group in improving urine retention,sleep quality score,effective rate was higher than the control group,anal border edema had no significant difference.No adverse reactions occurred in the two groups during treatment.Conclusion: Acupuncture at Chengshan Point and Shugu point in the treatment of pain after external stripping and internal ligation of mixed hemorrhoids is effective in clinical trials,which can significantly reduce the pain of patients,and can be widely used in the postoperative analgesia of mixed hemorrhoids.
