Objective: Through a retrospective analysis of 309 elderly patients with cerebral infarction aged ≥ 60 years old in the Department of Geriatrics of the Affiliated Hospital of Changchun University of Traditional Chinese Medicine from January 2019 to December2021,the distribution of syndromes,influencing factors and medication rules of cerebral infarction in elderly patients were clarified,providing ideas for TCM syndrome differentiation and treatment.Methods: Using the method of retrospective investigation and analysis,according to the patient ’s hospitalization data,organize and record the patient ’s relevant medical records,establish an Excel database,enter the data and proofread,and convert it into SPSS format after completion,and use SPSS22.0 software to analyze and discuss the relevant data,organize and summarize,summarize the rules,and form a preliminary understanding of the medication rules and influencing factors of this disease.Results:1.Among the 309 cases collected,the ratio of male to female was 1.22 : 1,and there was a correlation between gender,age,course of disease and solar term(P < 0.01).There were more male patients in the hospitalized patients,most of them were between 70 and 75 years old,the course of disease was mostly 1-5 years,and the number of patients admitted to the hospital was the most.There were differences in the admission season(P < 0.05),and the patients were mostly in summer.There was a correlation between smoking and drinking(P < 0.01).Smoking and drinking were mainly male.The common comorbidities in elderly patients with cerebral infarction were hypertension,coronary heart disease,dyslipidemia,and diabetes.Hyperuricemia was different in gender(P < 0.05),and male patients were more than female patients.2.The distribution of syndrome types in elderly patients with cerebral infarction was qi deficiency and blood stasis syndrome > wind phlegm and blood stasis syndrome > yin deficiency and wind stirring syndrome > phlegm heat and fu-organ excess syndrome,and there were significant differences in syndrome types(P < 0.01).The performance of elderly patients with cerebral infarction was mainly qi deficiency and blood stasis syndrome,and there was no difference in gender between each syndrome type(P > 0.05).Among the symptoms at admission,limb activity is the most common,and the tongue and pulse manifestations are basically consistent with the distribution of syndromes.3.Among the patients,there were significant differences(P < 0.01)in years of good fortune,dominant spirit,guest spirit and combination of good fortune.Born in the water transport less than the year,Taiyin wet Tuzhu,Taiyin wet Tusi day,the sun cold water in the spring,when the luck of the day most patients.There was no difference in luck,dominance and courtesy(P > 0.05).4.A total of 315 prescriptions of traditional Chinese medicine were included in this study.A total of 243 drugs were used,and 85 drugs were used more than 15 times.Among them,the top 10 drugs with the highest frequency were angelica,astragalus,chuanxiong,zhigancao,chishao,dilong,fuling,fried peach kernel,safflower and red koji.Combined with the distribution of syndromes,using SPSS system cluster analysis,commonly used Chinese medicine can be clustered into four categories.Qi deficiency and blood stasis syndrome is mainly tonifying qi and activating blood;the treatment of wind phlegm and blood stasis syndrome is based on the method of regulating qi and resolving phlegm,dispelling wind and inducing resuscitation;the treatment of phlegm-heat fu-viscera excess syndrome is mainly based on clearing heat and purging fire,dredging fu-viscera and turbidity.In the treatment of Yin deficiency and wind stirring syndrome,nourishing Yin and Qi,promoting blood circulation and removing wind should be the main methods.Conclusion:1.In this study,there were more males than females in middle-aged and elderly patients with cerebral infarction.The patients admitted to hospital were related to seasons and solar terms.The top related risk diseases were hypertension,coronary heart disease and dyslipidemia.There are differences between men and women in smoking,drinking and hyperuricemia.2.the treatment area included in the elderly patients with cerebral infarction Qi deficiency and blood stasis syndrome is the most common,patients often due to physical activity involvement and hospital treatment,more performance for cardiovascular and cerebrovascular disease.3.The occurrence of elderly patients with cerebral infarction in the study was associated with the year of birth luck,dominant Qi,guest Qi and luck.4.The clinical application of traditional Chinese medicine in elderly patients with cerebral infarction is consistent with the distribution characteristics of syndrome types.In the process of clinical diagnosis and treatment,the principle of syndrome differentiation and treatment should be followed,and the corresponding treatment prescription should be given flexibly according to the characteristics of patients ’ syndromes. |
