Study On Clinical Characteristics And TCM Constitution Distribution Of Patients With Insomnia And Functional Dyspepsia

Objective: To analyze the clinical characteristics of the patients with insomnia and functional dyspepsia,and to study the distribution of TCM Constitution of the patients with insomnia and functional dyspepsia,to sum up the common and common clinical features and susceptible constitution of this disease.Methods: A questionnaire survey was conducted among 163 patients who 2021 the comorbidity of insomnia and functional dyspepsia from December to October 2022 at the Insomnia Clinic and Endoscopy Department of the First Clinical Hospital of Jilin Academy of Traditional Chinese medicine,the general information and clinical characteristics of the patients were collected by using the questionnaire for comorbidity of insomnia and functional dyspepsia(self-designed),PSQI and ISI were used to assess the sleep quality and the severity of insomnia in the last month.Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD)and Somatic Self-rate Scale(SSS)were used to assess anxiety,depression and somatization symptoms the severity of the main FD symptoms was judged by the frequency judgment table of functional dyspepsia symptoms,and the TCM Constitution of the patients was judged by the TCM Constitution classification and judgment(ZYYXH/T157-2009).According to the contents of the questionnaire,the database was established with the software of EPI data 3.1,and the collected data were analyzed with the software of SPSS 25.0,including descriptive statistics,non-parametric test,chi-square test and correlation analysis,multi-category Logistic regression and other row data analysis.Results: 1.163 patients were included,including 63 male patients(38.65%)and 100 female patients(61.35%).The ratio of male to female was 1:1.59.The distribution of different gender samples in different age groups was significant(P < 0.05).Different exercise frequency had influence on the severity of FD symptoms(P<0.05).2.The most common clinical symptom of insomnia was difficulty in falling asleep,the most common clinical symptom of functional dyspepsia was early satiety and discomfort,the most common accompanying symptom was upset,followed by dry mouth and fatigue.There was no significant difference in clinical manifestation between different ages and sexes(P>0.05).3.There was a positive correlation between the scores of PSQI and HAMD,HAMA,SSS.The scores of FD symptoms were positively correlated with ISI(P<0.05).The scores of FD symptoms were positively correlated with HAMA,HAMD and SSS(P <0.05).4.According to the statistical analysis of the distribution of TCM Constitution,there were 38 cases(23.313%)of qi-stagnation constitution,36 cases(22.086%)of yang-deficiency constitution,27 cases(16.564%)of qi-deficiency constitution,16 cases(9.816%)of peaceful constitution and 16 cases(9.816%)of yin-deficiency constitution,there were 12 cases of damp-heat constitution(7.362%),11 cases of phlegm-dampness constitution(6.748%),6 cases of blood stasis constitution(3.681%)and 1 case of special constitution(0.613%).There were significant differences in physical fitness among different age groups(P<0.05).There was significant difference in sleep quality among different constitutions(P < 0.05),but no significant difference in the severity of FD symptoms among different constitutions(P>0.05).Conclusion: 1.Most of the patients with insomnia and FD were female,and the age distribution was mainly middle-aged and elderly.The most common clinical manifestations are difficulty in falling asleep,the most common symptoms of dyspepsia are early satiety and discomfort,accompanied by symptoms commonly found in upset,dry mouth,fatigue and so on.The frequency of exercise had an effect on the severity of FD symptoms.2.The severity of insomnia and FD symptoms was positively correlated with the severity of anxiety,depression and somatization.The severity of insomnia was positively correlated with the scores of FD symptoms.3.In the TCM Constitution distribution of insomnia and FD comorbidity,the main constitution is biased constitution,qi-stagnation constitution,yang-deficiency constitution and qi-deficiency constitution account for more.The distribution of constitution is related to the age factor,and there are differences between different age groups,regardless of gender.Different TCM constitution of patients with different severity of insomnia,Yang deficiency,blood stasis,Qi stagnation may be susceptible to their insomnia constitution.
