Objective:By observing the clinical efficacy of "cervical multi-point and multi-direction adjustment manipulation" in the treatment of atlantoaxial joint disorders,this study preliminarily discussed the therapeutic basis and mechanism of this therapy by comparing it with conventional massage manipulation,so as to provide an effective and safer treatment plan for the clinical diagnosis and treatment of atlantoaxial joint disorders.Methods:The collected cases in this study were all from 72 patients meeting the diagnostic criteria for atlantoaxial joint disorder who visited the massage clinic of Yunnan Hospital of Traditional Chinese Medicine,the Massage clinic of Dianchi Resort Hospital,and the Second Affiliated Hospital of Yunnan University of Traditional Chinese Medicine from December 2021 to December 2022.Statistical software spss26.0 was used to generate random numbers and 72 samples were equally divided into experimental group(36 cases)and control group(36 cases).The experimental group was treated with "cervical vertebra multi-point and multi-direction adjustment manipulation",while the control group was treated with "atlantoaxial articular instability manipulation" in Massage Therapy",the textbook of the 13th Five-Year Plan published by China Traditional Chinese Medicine Press in 2016 and compiled by Professor Fan Binghua.Before treatment,all patients were filled in the simplified McGill Pain scale,and MCU instrument was used to test the maximum isometric contractile force of neck forward flexion and extension,and the range of motion of neck forward flexion and extension,left and right rotation and left and right lateral flexion.In the course of the experiment,a total of 7 cases dropped out of the experiment due to various reasons,all of which met the shedding criteria or termination criteria,among which 3 cases fell off in the experimental group and 4 cases fell off in the control group.The number of patients who completed the required course of treatment was 65.From the day of inclusion,patients in both groups received 10 manipulative treatments,each treatment lasted about 20 minutes,once every other day.After the treatment course or the patient’s symptoms disappeared,the simplified McGill pain scale was filled in and the MCU instrument was used to test the neck flexor strength and neck range of motion.The obtained data were statistically analyzed and processed by statistical software spss26.0,and the efficacy of the two methods were compared to draw the experimental conclusion.Results:(1)After statistical analysis of gender,course of disease,age,score of simplified McGill Pain Scale,neck flexion and extension muscle strength measured by MCU and neck range of motion of the two groups of patients before treatment,there was no statistical difference in the data(P>0.05),indicating that there was no difference between the two groups of patients before treatment and they were comparable.(2)The efficacy of the simplified McGill Pain Scale score(including PRI,VAS,and PPI)was compared.The two groups were compared before and after treatment,and the data difference was statistically significant(P<0.05).The score of the two groups after treatment was compared between the two groups,and the data difference was also statistically significant(P<0.05).This indicated that both the treatment methods of the experimental group and the control group could relieve patients’ pain,but the treatment group was more effective in relieving the pain of patients with atlantoaxial joint disorder than the control group.(3)For the analysis of neck flexion and extension muscle strength and neck range of motion tested by MCU,intra-group comparison was made between the two groups before and after treatment,and the data difference was statistically significant(P<0.05);The data between the two groups after treatment were compared,and the difference(P<0.05)was also statistically significant.This indicated that both treatments could improve cervical flexion and extension muscle strength and range of motion in patients with atlantoaxial joint disorder,but the experimental group was more effective than the control group in alleviating cervical flexion and extension muscle strength and range of motion in patients with atlantoaxial joint disorder.(4)In the evaluation of the efficacy of the patients,of the 33 patients in the experimental group,24 were cured,9 were effective,and 0 were ineffective.The cure rate reached 72.7%.Among the 32 patients in the control group,8 were cured,and 24 were effective,with a cure rate of 25.0%.The test data were analyzed by the rank sum test,and the curative effect of the two groups was statistically different(P<0.05).This also showed that the clinical cure rate of the experimental group was significantly better than that of the control group.Conclusion:(1)Cervical spine multi-point and multi-direction adjustment manipulation and conventional massage manipulation can effectively treat atlantoaxial joint disorder,but the experimental group is better than the control group in reducing pain and neck muscle strength and motion.(2)The overall cure rate of cervical spine multi-point and multi-direction adjustment manipulation is higher than that of conventional massage manipulation;This indicates that cervical vertebra multipoint and multi directi on adjustment technique has high promotion value in clinic. |
