Clinical Observation Of Qingfei Ditan Tongjiang Decoction In The Treatment Of Acute Exacerbation Of Chronic Obstructive Pulmonary Disease(Phlegm-heat Constraint In The Lung Syndrome)with Constipation

Objective:To observe the clinical efficacy and safety of self-made Qingfei Ditan Tongjiang formula in the treatment of AECOPD(phlegm-heat constraint in the lung syndrome)with constipation.Methods:In this study,36 patients with acute COPD with constipation due to phlegm heat stagnation syndrome were selected as the treatment group and the other 36 patients as the control group.The control group(conventional western medicine+Maren capsule)and the treatment group(conventional western medicine+self-made Qingfei Ditan Tongjiang formula)were treated for 4 weeks as a course of treatment.The clinical symptoms,TCM syndrome efficacy,FEV1% of lung function and CAT score of the subjects before and after treatment were observed,and the relevant treatment indicators and safety indicators were recorded.The recorded results were compared and analyzed by statistical methods,and the efficacy and safety were scientifically evaluated.Results:1.TCM syndrome analysis: the total effective rate in the treatment group was 87.50%,significantly higher than 53.12% in the control group(P<0.05).There was no significant difference between the two groups in the single score of expectoration,bad breath and yellow urine(P>0.05),and the improvement of TCM syndrome efficacy in the treatment group was better than that in the control group(P<0.05).2.Analysis of the curative effect of CAT score: the total effective rate of the treatment group was 84.38%,which was significantly higher than 50.00% of the control group(P <0.05).3.FEV1% curative effect of lung function: the total effective rate of the treatment group was 78.13%,and that of the control group was 46.88%.Compared with the two groups,the treatment group was significantly higher than that of the control group(P < 0.05).4.Blood routine and blood oxygen saturation curative effect analysis: the blood routine(WBC,RBC,HB)and blood oxygen saturation of the two groups were significantly improved after treatment(P < 0.05),and the treatment group was more significant than the control group(P < 0.05).5.Analysis of safety indicators: in the clinical study and observation,all safety indicators of patients before and after treatment were normal without adverse reactions,and the safety of this clinical trial was guaranteed.Conclusion:1.The clinical efficacy of self-made Qingfei Ditan Tongjiang decoction combined with conventional western medicine in the treatment of elderly patients with acute exacerbation of COPD(phlegm heat stagnation lung syndrome)complicated with constipation is significantly better than that of conventional western medicine combined with Maren capsule.2.The treatment of integrated traditional Chinese and Western medicine can reduce the TCM syndrome score and CAT score of elderly patients with acute COPD(phlegm heat stagnation lung syndrome)complicated with constipation,effectively improve the FEV1%of lung function,relieve the clinical symptoms of patients with phlegm heat stagnation lung syndrome,and ensure the safety,which is worthy of further clinical exploration and development.
