| Purpose By collecting the medical records of children with allergic rhinitis in Kunming,this paper analyzes the incidence factors,TCM syndrome types and the distribution characteristics of syndromes,and discusses the correlation between the incidence factors and TCM syndrome types,so as to provide reference for clinical syndrome differentiation and prevention and treatment of the disease from various incidence factors.Method Through epidemiological investigation and research,the medical records of children ≤14 years old who were treated in the outpatient and inpatient departments of the First Affiliated Hospital of Yunnan University of Traditional Chinese Medicine from July 2021 to March 2023 and who met the diagnostic criteria for allergic rhinitis were collected by means of questionnaires.The database was established,and SPSS27.0 statistical software was used for statistical processing to analyze the distribution and correlation of the incidence factors,TCM syndrome types and syndromes.Result:A total of 200 cases with complete information were collected in this study.The basic information is as follows:1.General situation:The source of cases is mainly concentrated in 171 cases(85.5%)in the main urban area of Kunming,with the ratio of male to female being about 1.67:1.The age of treatment is mainly 100 cases(50.0%)at school age,the minimum age of year(month)is 7 months,the maximum age is 14 years,the course of disease is 1-3 years,81 cases(40.5%),the high incidence season is spring,autumn,and winter,the high incidence period is mainly morning and night,and the duration and severity of disease are mainly intermittent and mild.2.Distribution of relevant influencing factors:In this study,114 children(57.0%)had a family history of allergic rhinitis,69(34.5%)had a history of allergy,92(46.0%)had a past history(including the history of tic disorder,allergic dermatosis,pulmonary diseases,etc.),92(46.0%)had family members smoking,111(55.5%)were mainly breast-fed,and 146(73.0%)had supplementary food at the age of 6 months.3.Distribution of TCM syndrome types and syndrome groups:In this study,the TCM syndrome types were divided into 86 cases(43.0%)of dryness evil hurting lung syndrome,51 cases(25.5%)of wind-heat syndrome of lung meridian,27cases(13.5%)of lung-qi deficiency syndrome,20 cases(10.0%)of damp-heat accumulating lung syndrome,13 cases(6.5%)of wind-cold attacking lung syndrome,and3 cases(1.5%)of kidney-yang deficiency syndrome.According to the frequency statistics of the four diagnostic data of traditional Chinese medicine,two types of syndromes were obtained after screening the common syndromes with a frequency of more than 30% and hierarchical cluster analysis.The indicators of the first type of syndrome were: clear voice,heavy voice,mucous membrane hyperemia and moist,mucous membrane hyperemia and dry,lower turbinate slightly swollen,lower turbinate swelling,pharyngeal gorge without special,pharyngeal gorge hyperemia,dry nose,clear nose,itchy eyes,cough with less sputum,dry stool,red tongue and thin white coating,Syndrome differentiation is the syndrome of dry evil hurting lung.The indicators of the second type of syndrome group are: ruddy face,moist hair,red and moist lips,white eyes,blue air pool,nasal congestion,sneezing,itchy nose,normal diet,normal sleep,normal urination,normal stool,and pulse floating number.The syndrome is identified as lung meridian wind-heat syndrome.4.Relationship between influencing factors and TCM syndrome types:Correlation analysis showed that the time of adding supplementary food and allergy history had an impact on the distribution of TCM syndrome types of allergic rhinitis in children(P<0.05).Gender,age stage,region,premature delivery,term condition,delivery mode,birth weight,asphyxia history,pregnancy disease history,maternal diet during pregnancy,family allergic rhinitis history,nutritional supplement,past history,feeding mode,parents’ education background,parents’ age,smoking status of family members,duration,disease grading,time and season of onset had no significant impact on the distribution of TCM syndrome types of children with allergic rhinitis(P>0.05).Conclusion1.This study found that the prevalence of childhood allergic rhinitis in Kunming is high in school-age boys,with family history of allergic rhinitis,past history(including tic disorder,allergic dermatosis,pulmonary disease history,etc.),allergy history,and family smoking as high-risk factors for childhood allergic rhinitis.And allergic rhinitis is also a high risk factor for Tourette disease and other lung diseases.2.The incidence and syndrome types of allergic rhinitis in children in Kunming are affected by seasons,local climate,children’s physique and physiological characteristics,so the syndrome of lung injury caused by dryness,wind-heat syndrome of lung meridian,and lung-spleen deficiency syndrome are more common.The time of adding supplementary food and allergy history have an impact on the distribution of TCM syndrome types.3.Based on the results of this study,doctors should do a good job in health education to avoid or reduce high-risk factors.In the aspect of differentiation and treatment of traditional Chinese medicine,we should take measures according to people,time and local conditions,take into account the symptoms,dispel pathogenic factors and consolidate the root cause.On the basis of the regulation and treatment of the lung and spleen,we should give the treatment of dredging the orifices,moistening the dryness,clearing the heat and dispelling the wind. |
