Clinical Observation Of Wuzixiaoyao San Combined With Acupuncture In The Treatment Of PCOS Complicated With Depression And Anxiety

ObjectiveThis study was conducted to evaluate the effectiveness of the treatment regimen in improving depression and anxiety,menstrual cycle and sex hormone levels in patients with polycystic ovary syndrome combined with depression and anxiety state by combining Wuzixiaoyao San with acupuncture,and to evaluate its clinical efficacy,so as to provide reference clinical information and evidence for the treatment of patients with polycystic ovary syndrome combined with depression and anxiety state.MethodsOne hundred and thirteen patients with PCOS who met the inclusion criteria and were treated at the Department of Gynecology,Xiamen Hospital of Traditional Chinese Medicine between01/2022 and 12/2022 were selected and randomly divided into Chinese medicine group,needle medicine group,and western medicine group.The three groups were treated for 1month as a cycle,and after 3 months of continuous treatment,the combination of Wuzixiaoyao San and acupuncture was evaluated by observing the SAS score,SDS score,TCM syndrome score,and the detection of T,LH,FSH,E₂and other sex hormones.The clinical efficacy of the treatment of polycystic ovary syndrome combined with depression and anxiety was evaluated by observing SAS scores,SDS scores,TCM symptoms scores,T,LH,FSH,E₂and other sex hormones.Results1.SAS score and SDS score:After treatment,the SAS score of the three groups was significantly lower than that before treatment,and the difference was statistically significant（P<0.05）.The SAS scores of the needle medicine group were significantly lower than those of the Chinese medicine and western medicine groups after treatment,and the difference was statistically significant（P<0.05）.The SDS scores of the three groups were significantly lower after treatment than before treatment,and the difference was statistically significant（P<0.05）.The SDS scores of the needle medicine group were significantly lower than those of the Chinese medicine group and the western medicine group after treatment,and the difference was statistically significant（P<0.05）.2.TCM evidence points:the TCM evidence points of the three groups were significantly lower after treatment,and there was a significant difference in the comparison within the groups（P<0.05）.After treatment,the TCM syndrome points of the needle and medicine group were significantly lower than those of the Chinese medicine and western medicine groups,and the difference was statistically significant（P<0.05）.The correlation analysis of SAS score,SDS score and TCM symptom score indicated that SAS score and SDS score were positively correlated with TCM symptom score and menstrual cycle（P<0.05）.3.Comparison of sex hormone efficacy:（1）T was significantly lower in all three groups after treatment than before treatment,and the difference was statistically significant（P<0.05）,and the decrease in T level in the needle medicine group and Chinese medicine group was significantly better than that in the western medicine group,and the difference was statistically significant（P<0.05）.（2）LH was significantly lower in all three groups after treatment than before treatment,and the difference was statistically significant（P<0.05）,and the decrease in LH level in the needle medicine group was significantly better than that in the Chinese medicine group and the western medicine group,and the difference was statistically significant（P<0.05）.（3）The FSH was significantly lower in the three groups of patients after treatment than before treatment,and the difference was statistically significant（P<0.05）,and there was no significant difference in the FSH levels in the three groups（P>0.05）.（4）The LH/FSH was significantly lower in the three groups of patients after treatment compared with that before treatment,and the difference was statistically significant（P<0.05）.The decrease of LH/FSH in the needle medicine group was significantly better than that in the Chinese medicine and western medicine groups,and the difference was statistically significant（P<0.05）.（5）E₂was significantly higher in the three groups after treatment than before treatment,and the difference was statistically significant（P<0.05）,and there was no significant difference in E₂levels between the three groups after treatment（P>0.05）.（6）Patients in the Chinese medicine group had no significant change in PRL after treatment compared with that before treatment（P>0.05）,and patients in the needle medicine and western medicine groups had a decrease in PRL after treatment compared with that before treatment,and the difference was statistically significant（P<0.05）,and there was no significant difference in PRL level between the three groups（P>0.05）.（7）The difference between pre and post SAS scores was positively correlated with the difference between pre and post LH（P<0.05）.（8）The pre-post difference of SDS score was positively correlated with the pre-post difference of T,pre-post difference of LH,pre-post difference of FSH,and pre-post difference of LH/FSH（P<0.05）.4.Comprehensive efficacy comparison:the comprehensive efficacy of the Chinese medicine group was 73.68%,the comprehensive efficacy of the needle medicine group was 86.11%,and the comprehensive efficacy of the western medicine group was 61.54%.The comprehensive efficacy of the needle and medicine group was significantly better than that of the traditional Chinese medicine group and the western medicine group,and the difference was statistically significant（P<0.05）.Conclusion1.Wuzixiaoyao San combined with acupuncture treatment can significantly reduce SAS score and SDS score in patients with polycystic ovaries combined with depression and anxiety,and significantly improve late menstruation,irregular menstruation,and improve the manifestation of kidney deficiency and liver depression,and the efficacy is better than that of the Chinese medicine group and the western medicine group.2.Wuzixiaoyao San combined with acupuncture treatment can significantly improve the TCM symptoms,and the symptoms of menstrual cycle,menstrual flow,lumbar and knee tenderness,dizziness and tinnitus,breast swelling and pain after treatment were significantly improved compared with the Chinese medicine group and western medicine group.positive correlation existed between SAS and SDS scores and TCM symptom scores,indicating that the higher the TCM symptom scores and the longer the menstrual cycle,the higher the degree of anxiety and depression.3.Wuzixiaoyao San combined with acupuncture treatment significantly reduced T level,reduced LH level,reduced FSH level,reduced LH/FSH ratio,increased E₂level,and reduced PRL level.The efficacy of reducing T level was better than that of the Western medicine group,and the efficacy of reducing LH level was better than that of the Chinese medicine group and Western medicine group,and the efficacy of reducing LH/FSH ratio was better than that of the Chinese medicine group.The difference in SAS scores before and after treatment was positively correlated with the difference in LH,and the difference in SDS scores before and after treatment was also positively correlated with the difference in T,LH,and FSH,suggesting that the relief of the state of depression and anxiety was correlated with the improvement of sex hormone indexes.
