On The Optimization Of Urban Community Health Service Center In The Construction Of Medical Consortia

On March 11,2021,the Fourth Session of the 13 th National People’s Congress voted on the Resolution on the 14 th Five-Year Plan for National Economic and Social Development and the Outline of Vision 2035 Visionary Goals.In the 14 th Five-Year Plan,"accelerating the construction of a hierarchical medical treatment system and actively developing medical consortia" is an important part of deepening the reform of the medical and health system.China’s population is huge,and there is a long-standing shortage of quality medical and health resources,unbalanced distribution and low utilization,and the problem of "difficult and expensive access to medical care" has not been effectively solved.The few tertiary hospitals in each region are often overcrowded,while the widely distributed and more convenient primary health care institutions do not have many patients,creating a stark contrast between the two.To enrich quality medical and health resources,strengthen resource allocation capacity as well as improve resource utilization efficiency,the key lies in improving the intrinsic service capacity of primary medical and health care institutions.Based on this,it is of great theoretical and social significance to take primary health care institutions as the research object and study how to further optimize their role in the construction of regional medical consortia.Based on public goods theory,synergy theory,and stakeholder theory,this thesis focuses on improving the intrinsic service capacity of primary health service institutions by using literature research method,on-site interview method,and questionnaire survey method to investigate the operation status of community health service centers under the medical consortium model.The current situation,problems and causes of the operation of community health service centers in urban areas of L district under the medical consortium model were analyzed in depth,and relevant countermeasures were proposed.Through field research and summary of four urban community health service centers in L district,we understood,collected and summarized the problems in the development of community health services in L district,such as insufficient hardware construction,insufficient development of basic public health services,lack of talents,emphasis on treatment rather than prevention,insufficient trust of residents,and ineffective implementation of the work of the medical consortia.The main reasons for this are the lack of awareness of the importance of primary health service work,insufficient investment in basic public health,imperfect personnel and talent system,inconsistency between the performance assessment orientation and value orientation of the center,mismatch between the service level of the center and residents’ expectations,and insufficient supporting support for the medical association.Through the analysis of the causes of the problems,countermeasures and suggestions such as strengthening the leading position and supporting role of the government,top-level design to promote the participation mechanism of diversified subjects,enriching the construction of the talent team,promoting family doctor contracting services,and giving full play to the advantages of community service centers themselves are proposed,and based on the common problems and improvement measures of each case,a universal strategy to promote the development of community health services in L district is summarized and refined,with a view to provides reference for the decision of community health service development in L district and case experiences for the development of community health service centers in other regions.
