Objective: Breast cancers are classified according to ER,PR,HER2 and Ki-67 expression on the basis of molecular profiling,however,as far as breast cancer individuals are concerned,it may be inherently the case that there are subcellular subpopulations with different molecular profiles.In this study,we investigated the presence and proportion of basal like subcellular populations in HR + / HER2-breast cancer and investigated the relationship between the presence and prognosis of basal like subcellular populations.Research methods: In this study,we confirmed the presence of basal like subpopulations in HR + / HER2-breast cancer by measuring the expression of basal like markers EGFR,CK14,and CK5 / 6 in hormone receptor(HR)positive HER2 negative breast cancer tissue.1.Clinical case data: pathologic paraffin sections of 180 patients with pathologically confirmed primary breast cancer after surgical resection were collected from April 2010 to November 2016 at the Department of breast surgery,the First Affiliated Hospital of China Medical University.2.The expression of basal like basal markers EGFR,CK14,and CK5 / 6 was assessed in the pathological tissue of 180 breast cancer patients:(1)cellular level:immunohistochemistry was performed and(2)molecular level: paraffin samples of HR +/ HER2-breast cancer tissues were selected,Transcriptome sequencing(RNA SEQ)was performed to examine the whole genome expression profiles in paraffin samples of breast cancer tissues from the three cases.3.Statistical analysis:(1)the data were analyzed by SPSS 26.0: the relationships between the expression of basal like subpopulations of patients with different prognosis and clinical data(age,menstrual status,tumor size,axillary lymph node metastasis,Ki-67 expression,histological grade,clinical stage)were analyzed by chi square test;Univariate and multivariate survival analyses were performed using Cox regression models;(2)Transcriptome sequencing results were analyzed by pam50 using R 4.2.1;(3)Graphpad pris9 software was used to draw survival curves according to the Kaplan Meier(K-M)method,and log rank test was used to compare the prognostic effects of high and low risks of the prediction models on DFS;Examination of the level of α= 0.05,P < 0.05 differences were considered statistically significant.Result: 1.In this study,the overall positive expression rate of basal like basal marker EGFR was 69.45%(125 / 180),the overall positive expression rate of CK14 was 42.22%(76 / 180),and the overall positive expression rate of CK5 / 6 was 15%(27 / 180);The overall expression rate was 34.44%(62 / 180)in basal like subcellular populations in which EGFR and CK14 or CK5 / 6 were simultaneously positive,and 65.56%(118 / 180)in non basal like subcellular populations.2.In HR+/HER2 invasive breast cancer without chemotherapy,DFS in patients with ER ≤ 10% was significantly lower than that in patients with ER>10%(P=0.0003,P<0.05)3.The expression of basal like subpopulations was significantly different between patients with poor and good prognosis in HR + / HER2-breast cancer(P = 0.039;P < 0.05).4.In this study,the expression ratio of basal like subpopulations of HR + / HER2-breast cancer was correlated with histological grade,and the differences were statistically significant(P < 0.0001;P < 0.05);However,there was no correlation between the expression rate of basal like subcellular populations and patient age,menstrual status,tumor size,axillary lymph node metastasis,Ki-67 expression and clinical stage,and the differences were not statistically significant(P > 0.05).5.The expression of basal like subpopulation of HR + / HER2-breast cancer was found to be associated with patient DFS(HR 2.099,95% CI 1.026-4.294,P = 0.0423)Conclusion: 1.HR+/HER2 invasive breast cancer without chemotherapy,patients with low ER expression rate have worse prognosis;2.HR+/HER2 invasive breast cancer has a certain proportion of Basal like sub cell population,which may be the cytological reason for the difference in prognosis;3.The expression of HR+/HER2-invasive breast cancer Basal like subpopulation is associated with higher histological grade;4.The prognosis of patients with HR+/HER2 invasive breast cancer with Basal like subcellular expression is worse,and it has an independent predictive effect. |
