ObjectiveTo observe the distinguish pathologic expression and clinical characteristics and explore the prognosis and prognostic factors of basal like breast cancer by studing the data of48basal like breast cancer and198non-basal like breast cancer.Methods246female patients with newly diagnosis of infiltrating ductal breast carcinoma from Jan2006to Oct2008in Qingdao Municipal Hospital were analysed retrospectively. By observing the special pathologic morphology and detecting the estrogen receptor (ER), progestrogen receptor (PR), human epidermal growth factor receptor2(HER2/C-erbB-2), epidermal growth factor receptor (EGFR) and cytokeratin5/6(CK5/6) of the246patients,48patients were diagnosed as basal like breast cancer with the standard of having distinguish pathologic characteristics and with the negative expression of ER, PR, Her-2(C-erbB-2) yet with the positive expression of EGFR and CK5/6. All the other patients were divided into the group of non-basal like breast cancer. By contrasting the data of the two groups, we analysed the distribution features in the population of basal like breast cancer, and analysed its prognosis and prognostic factors.ResultsIn this study,48patients were diagnosed as basal like cancer, which was24.2%of all the infiltrating duct cancer patients during the same time. The majority performance of basal like breast cancer is such as lumps, fluke, nested, and thick banded structure. Most cell nucleus of basal like breast cancer was round, oval, coarse, nuclear chromatin, a majority of nucleoli.These appearances were common in the group of basal like breast cancer as tumor diameter being more than5cm, high cell grade, vessels invasive, Ki67positive expression being more than80%and patients being premenstruation. The recurrence and mortality rate of basal like breast cancer was50.0%, while it was13.6%of non-basal like breast cancer.There is significant difference. It was16.7%of basal like breast cancer and5.6%of non-basal like breast cancer if we took mortality into account singly. The difference is also significant. In brief, the prognosis of basal like breast cancer is worse than non-basal like breast cancer. The prognosis of patients who was being less than35years old, with high cell grade, having positive axillary lymph node and vessel invasive was worse than the patients without the same situation in the group of basal like breast cancer. ConclusionBasal like breast cancer is a special sub-type of breast cancer, which appearing distinguish pathologic and population-based distribution characteristics. This may help to diagnose basal like breast cancer more exactly. The age of patients, cell grade, state of lymph node and state of vessel invasive may be helping to evaluate the prognosis of basal like breast cancer.In common, The prognosis of patients who was being less than35years old, with high cell grade, having positive axillary lymph node and vessel invasive was worse than the patients without the same situation in the group of basal like breast cancer. Therefore, the treatment and follow-up after diagnosis of this kind of breast cancer should be intensified. |
