Objective: To explore the association of platelet distribution width(PDW),fibrinogen(FIB)and coronary heart disease(CHD)and the degree of coronary disease.Further explore whether PDW combined with FIB can be used for early diagnosis and evaluation of the extent of CHD.Methods:207 CHD patients admitted to Cardiology of the Fifth Affiliated Hospital of Xinjiang Medical University from April 2021 to July 2022,all underwent coronary angiography,and 134 patients with CHD were selected as the case group to calculate the Gensini score of case group;51 <20,subgroup 20-40 and 44> 40;73 patients with coronary angiography suggesting non-CHD were selected as the control group.General data of each group were collected and early morning fasting venous blood measurements for PDW and FIB were collected.The indicators different in univariate analysis were used as independent variables,and whether to combine CHD as the dependent variable for Logistic regression analysis yielded independent influencing factors of CHD.The value of the presence of CHD was evaluated by the subject operating characteristic curve(ROC).The correlation with the degree of coronary artery lesion(Gensini score)was analyzed by Spearman correlation.Results: Statistically significant difference(PDW: OR = 1.330,95%CI 1.044~1.694,P<0.021;FIB:OR=3.076,95%CI1.507~6.279,P<0.002);ROC plotted with PDW and CHD,The area under the curve AUC was 0.674,SE=0.039,95%CI 0.573-0.752,P<0.001,The optimal cutoff value was 12.55,The sensitivity was 50.7%,The specificity ratio was 74.0%.ROC with FIB and CHD shows that area under the curve AUC is 0.674,SE=0.039,95%CI 0.597-0.752,P <0.001,optimal cut-off value is 2.66,specificity is 42.5%,combine PDW with FIB index,AUC =0.715,SE=0.036,95%CI 0.644-0.785,P <0.001,sensitivity is 60.4%,specificity is 71.2%.The correlation analysis results indicate that there is a significant positive correlation between the Gensini score in the case group and PDW and FIB(PDW=0.346,P<0.001),FIB(r=0.418,P<0.001).Conclusion:PDW and FIB are independent risk factors for CHD and are positively correlated with the degree of coronary artery disease(Gensini score),which has certain reference value for CHD screening and evaluation of disease degree.The combination of PDW and FIB has a more significant clinical value for the diagnosis and evaluation of CHD.
