| Objectives:To observe curative efficacy of Chinese herbal fomentation bag combined with modified Yanghe decoction for treating ASO of lower extremity with syndrome of cold coagulation and blood stasis and to assess its feasibility and safety by comparing clinical symptoms,signs and blood indexes before and after treatment.Methods:A total of 50 patients who were diagnosed as ASO of lower extremity and classified as syndrome of cold coagulation and blood stasis according to TCM syndrome differentiation were selected,who were hospitalized in the Second Department of Peripheral Vascular Disease of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from March 2022 to August 2022.Fifty patients were randomly divided into the treatment group and the control group,with 25 cases in each group.The control group was given common treatment that the patients were given floium ginkgo extract and tertramethylpyrazine sodium chloride injection and pentoxifylline injection by intravenous drip while taking modified Yanghe decoction orally.On the basis of common treatment,the treatment group was also treated with Chinese herbal fomentation bag which was placed at the gastrocnemius muscles of both lower limbs.Patients’ Weizhong acupoints(BL40),Chengshan acupoints(BL57)and Sanyinjiao acupoints(SP6)were covered by Chinese herbal fomentation bag.The treatment period of two groups was 21 days.Quantitative scores of symptoms of ASO of lower extremity,ABI,skin temperature of shanks and insteps and blood indexes were observed and compared between two groups before and after treatment so as to evaluate efficacy and safety of Chinese herbal fomentation bag combined with modified Yanghe decoction for treating ASO of lower extremity.Results:After treatment,clinical symptoms,signs and blood indexes of two groups were significantly improved,which were compared with those before treatment(P<0.05).The overall response rate of the treatment group was 96.00% and that of the control group was76.00%.The curative efficacy of the treatment group was obviously better than that of the control group(P<0.05).In terms of clinical symptoms and signs,the treatment group was better than the control group in the improvements of skin temperature sense,pain,IC distance,IC remission time,ABI and skin temperature of shanks and insteps(P<0.05).In terms of blood indexes,the treatment group was better than the control group in the improvement of CRP(P<0.05),while there was no significant difference in the improvement of FIB between two groups(P>0.05).There was no adverse reaction in two groups during the treatment period.Conclusions:1.Chinese herbal fomentation bag combined with modified Yanghe decoction can alleviate clinical symptoms of ASO of lower extremity with syndrome of cold coagulation and blood stasis such as coldness of lower limbs and pain,can increase intermittent claudication distance,can improve ABI,can improve skin temperature of lower limbs and can reduce CRP,which has obvious clinical efficacy.2.Chinese herbal fomentation bag combined with modified Yanghe decoction has significant curative efficacy,high safety and strong feasibility for treating ASO of lower extremity with syndrome of cold coagulation and blood stasis,which is suitable to be promoted in clinical practice as an appropriate technique of traditional Chinese medicine. |
