Objective:To evaluate the sensitivity,specificity and clinical predictive value of Portable monitoring(PM)in the diagnosis of children with obstructive sleep apnea hypopnea syndrome(OSAHS)by synchronously applying PM and Polysomnography(PSG)for sleep monitoring in children with suspected OSAHS at night.Methods:In this study,110 children aged 4-16 with snoring or/and mouth breathing who were admitted into the Pediatrics Department of the First Affiliated Hospital of Xinjiang Medical University from January 2022 to January 2023 were included.The general data including age,gender,height,weight and body mass index were collected.Meanwhile,PM and PSG were used for all-night sleep monitoring,including apnea hypopnea index(AHI),respiratory events,total sleep time(TST),sleep latency,sleep efficiency,sleep awakening time,mean oxygen saturation(MSa O2),lowest oxygen saturation(LSa O2),mean pulse rates,fastest pulse rates and slowest pulse rates.Taking PSG as the gold standard for the diagnosis of OSAHS,subjects were divided into OSAHS group and simple snoring group,and OSAHS group was divided into mild group,moderate group and severe group.The sensitivity,specificity,positive likelihood ratio and negative likelihood ratio of PM in the diagnosis of OSAHS were calculated,and ROC curves were drawn.The differences between PM and PSG were compared to analyze the diagnostic value of PM in children with OSAHS.Results:1)The sensitivity,specificity,positive likelihood ratio and negative likelihood ratio of PM in the diagnosis of OSAHS were 96.20%,93.55%,14.91and 0.04,respectively;2)The sensitivity of PM in the diagnosis of mild,moderate and severe OSAHS was 82.61%,84.38%and 86.96%respectively,the specificity was 76%,84.38%and 90.91%respectively,the positive likelihood ratio was 3.44,5.40 and 9.57respectively,the negative likelihood ratio was 0.23,0.19 and 0.14 respectively,and the area under ROC curve was 0.975,0.990 and 0.979 respectively;3)There was no significant difference between PM and PSG(P>0.05).Conclusion:PM has high sensitivity and specificity in the diagnosis of OSAHS in children.AHI,TST,sleep latency,MSa O2,LSa O2,mean pulse rates,fastest pulse rates,slowest pulse rates,respiratory events,sleep efficiency and sleep awakening time of PM are consistent with those of PSG.PM can replace PSG in diagnosing and predicting children with suspected OSAHS,which can be used for preliminary screening of diseases.
