| Objective: To study the application of two hypoglycemic regimens in patients with newly diagnosed type 2 diabetes mellitus complicated with fatty liver,and to observe their effects on blood glucose and islet β-cell function.Methods: From November 2021 to September2022,110 newly diagnosed patients with type 2 diabetes mellitus complicated with fatty liver who visited the Department of Endocrinology,Kashi People’s Hospital,Xinjiang,were randomly assigned to Group 1 and Group 2,with 55 cases in each group.Group 1:application of pioglitazone metformin tablets,combined with SGLT-2i,GLP-1RAs,group2: application of metformin combined with SGLT-2i,GLP-1RAs.The two groups were treated for 16 weeks,and the general data indicators,fasting insulin(FINS),HbA1 c,FBG,and calculation of insulin resistance index(HOMA-IR)and islet β-cell function index(HOMA-IR)were observed before and after treatment in the two groups.Results: 1.Before treatment,the height,age,weight,SBP,DBP,BMI,HbA1 c,FBG,FINS,AST,ALT,HOMA-β index,HOMA-IR index and other indicators of the two groups of patients had no statistical difference(P>0.05)The baselines were comparable.2.Comparison of blood sugar indicators: There was no statistically significant difference between Group 1 and Group 2in FBG,FINS,and HbA1c(P>0.05).Taking HbA1c<6.5% as the target standard,after treatment,the HbA1 c target rate of group 1 was 46.8%;the HbA1 c rate of group 2 was34.88%,the difference was statistically significant(P<0.05).When HbA1c<7.0% was used as the target standard,after treatment,the HbA1 c target rate of group 1 was 57.4%;the HbA1 c rate of group 2 was 55.8%,the difference was statistically significant(P<0.05).3.Comparison of islet function: There was no significant difference in the levels of HOMA-βand HOMA-IR between the two groups after treatment(P>0.05).4.Comparison of general information;after treatment,the weight and BMI data of the two groups were 82(71.75,87.50)vs 73(68,82),29.76(27.08,32.73)vs 27.46(25.81,30.12),above(P< 0.05)the difference was statistically significant;compared with DBP,SBP,AST,ALT between the two groups after treatment,(P>0.05)the difference was not statistically significant.Conclusion: Both groups can improve the blood sugar index of patients,improve islet resistance,islet β-cell function,and increase the standard rate of glycosylated hemoglobin.The treatment plan of group 1 is relatively more beneficial in improving blood sugar index,insulin resistance index,and islet β-cell function index. |
