Objective: To investigate whether urine Alzheimer’s associated neurofilament protein(AD7c-NTP)alone is more advantageous in the diagnosis of Alzheimer’s disease(AD)than combined with interleukin-6(IL-6)and high-sensitivity C-reactive protein(Hs-CRP).Methods: 64 AD patients admitted to our hospital in the past three years were selected as the case group, and a control group of 64 healthy physical examinees who visited our hospital was also selected. The urine AD7c-NTP,serum IL-6,and serum Hs-CRP levels of both groups were tested, and the Mini-Mental State Examination(MMSE)questionnaire was completed. The case group was also evaluated with the Clinical Dementia Rating(CDR)questionnaire. The basic characteristics of the two groups were compared, and the results of serum IL-6,serum Hs-CRP,and urine AD7c-NTP levels were compared. The correlation between MMSE scores and serum IL-6,serum Hs-CRP,and urine AD7c-NTP levels in the case group were analyzed. The diagnostic value of urine AD7c-NTP alone and combined with serum inflammatory factors detection for AD patients was analyzed, using ROC curve to analyse. Results:Compared with the urine AD7c-NTP,serum IL-6,and serum Hs-CRP test results of the control group, both mild and moderate to severe AD patients had significant differences(all P<0.05).Through analysis,it was found that the urine AD7c-NTP,serum IL-6,and serum Hs-CRP levels of AD patients were negatively correlated with MMSE scores(all r<0),and the former two levels were positively correlated with the patient’s CDR score(both r>0).The urine AD7c-NTP level of AD patients was positively correlated with their serum IL-6 and serum Hs-CRP levels(r>0).Logistic regression analysis showed that the sensitivity and specificity of multi-index joint detection for the diagnosis of AD in AD patients were 76.6% and 96.9%,respectively,and the AUC curve area was 0.935.ROC curve analysis showed that the difference between the three indicators detected alone and in combination was statistically significant(P<0.05).Conclusion: The levels of AD7c-NTP in urine,IL-6 in serum, and Hs-CRP in serum of case group were significantly higher compared to the control group. The levels of AD7c-NTP in urine,IL-6 in serum, and Hs-CRP in serum of case group were negatively correlated with their cognitive level, while the levels of AD7c-NTP in urine and IL-6 in serum were positively correlated with the severity of the disease. The joint detection of AD7c-NTP in urine,IL-6 and Hs-CRP in serum has good diagnostic value, and the combination of these three tests is more suitable for further testing of suspected AD patients after initial screening. |
