Damage to memory, language, judgement, perspicacity, behavior and personality damage is the major clinical situation. Cognitive, ability of life and behavior deterioration, irritation and epilepsy may appear among the serious patients. It is a heavy burden for family and society. Besides cardiovascular disease,cancer and stroke, AD is the fourth-leading cause of death , and is attented in the world. Although it has made remarkable progress in the pathogenesy and clinical diagnosis of AD, we have no method to prevent and cure AD effectively by now. After longitudinal study, it is the key to achieve early clinical diagnosis of AD so that more effective therapy can be started earlier in the course of AD. In clinical practice, there are no specific biochemical markers for the diagnosis of AD; current criteria for the diagnosis of AD are still largely based on psychiatric test, which are still not unified. Affected brains of AD patients are characterized by the presence of senile plaques (SP) and neurofibrillary tangles (NFT), It is showed by studies that AD patients'neurofibrillary tangles contain lots of Alzheimer associated neuronal thread protein (AD7c-NTP) and the express of the AD7c-NTP is increasing in the brain of AD. There is AD7c-NTP in the urine of AD patients and its molecular mass is the same with the AD7c-NTP from cerebrospinal fluid(CSF). In this article, it is focused on what recent advances measuring AD7c-NTP in urine of AD , and evaluating its significance for the diagnosis of AD.Objective: To study the early diagnostic significance of improved method for determining urinary level of AD7c-NTP in Alzheimer disease.Subjects :Subjects with AD (n=40) and age-matched healthy control (HC, n=20)were included in this study. All AD patients were diagnosed according to the NINCDS-ADRDA (National Istitute of Neurologocal and Communicative and Stroke-Alzheimer's Disease and Related Disorders Association) critiaria and DSM-IVR(Diagnostic and Statistical Manual of Mental Disorders, fourth edition revised) criteria. Eighteen mild AD (19≦M MSE≦2 6, CDR=1)and Twenty-two moderate and severe AD (MMSE<19,CDR≧ 2) were included in AD group.Methods:middle piece of urina sanguinis was collected from subjects ; clear supernatant liquid of urine was separated by centrifuge at 4℃, kept around 4℃less than 2h,and measureted using ELISA.Results: Level of AD7c-NTP was significantly higher in the Total AD group than that in HC(p<0.05). The mean level of AD7c-NTP in the urine of mild AD group were higher than that in HC group(p<0.05). The mean level of AD7c-NTP in the urine of moderate- AD group were higher than that in mild AD group(p<0.05).Conclusion: The data suggest that urine AD7c-NTP is increased in AD,if improving the technique, it might be a potential biochemical tool for the diagnisis of AD. MCI is different from normal aging but associated with AD,so MCI is essential aspect for early diagnosis and early intervention therapy. Derived from the first part of the experiment that the level of AD7c-NTP was significantly higher in the Total AD group than that in HC, The mean level of AD7c-NTP in the urine of mild AD group were higher than that in HC group and The mean level of AD7c-NTP in the urine of moderate- AD group were higher than that in mild AD group.In view of this, this section studied the content of urine AD7c-NTP of MCI patients to explore the change in precursor status of AD and analysis of its pre-clinical diagnosis significance of AD, aims to achieve AD patients with early diagnosis, early treatment.Objective : To study the early diagnostic significance of improved method for determining urinary level of AD7c-NTP in Alzheimer disease.Subjects: Subjects with Mild cognitive Impairment (MCI, n=29) and age-matched healthy control (HC, n=20) were included in this study. All AD patients were diagnosed according to the critiaria of MCI made by Peterson .Methods: Middle piece of urina sanguinis was collected from subjects ; clear supernatant liquid of urine was separated by centrifuge at 4℃, kept around 4℃less than 2h,and measureted using ELISA.Results :There was no sigfinicant difference of the mean leavel of AD7c-NTPin urine between the MCI group and HC group.(p>0.05).Conclusion: AD7c-NTP of urine cant't be a potential biochemical tool for the diagnisis of MCI by this result. |
