Objective: To study and discuss the changes and clinical significance of platelet,neutrophil to Lymphoid ratio(NLR),platelet to lymphoid ratio(PLR)and immunoglobulin in children with Mycoplasma pneumoniae(MP)infection.A prediction model of Mycoplasma pneumoniae(MP)infection was established by using common serological detection indicators,and the value of the prediction model was evaluated.Finally,the serological detection indicators with the highest diagnostic value for MP infection were selected.Method: A total of 259 hospitalized children with respiratory tract infection from December 2020 to January 2023 were selected.According to MP-igm antibody test or antibody titer test,they were divided into MP positive group and MP negative group.SPSS 26.0 statistical software was used for univariate and Logistic regression multivariate analysis.Then the characteristic variables were screened by Lasso regression,and the R software was used to draw the nomogram model of the above characteristic variables.At the same time,the ROC curve was made,and the area under the curve(AUC)was calculated to evaluate the diagnostic value of the prediction model.Results:(1)There was no statistical significance in gender and age difference between MP positive group and MP negative group in univariate analysis(P>0.05),the levels of red blood cell count(RBC),platelet count(PLT),mean red blood cell volume(MCV)neutrophil count(Neu #),neutrophil to Lymphoid ratio(NLR),platelet to lymphoid ratio(PLR),IgG and IgM in MP positive group were higher than those in MP negative group.Average erythrocyte hemoglobin content(MCH)and erythrocyte distribution width(RDW-SD)levels were lower than those of negative group,and the differences were statistically significant(P<0.05);(2)Correlation analysis of 10 indexes(RBC,PLT,MCH,MCV,RDW-SD,NEUT#,IgG,IgM,NLR,PLR)screened by single factor analysis showed that the correlation between NEUT# and PLT was the strongest,and the absolute value of correlation coefficient was 0.684,which was positive.(3)When λ value was 000,six characteristic indexes(PLT,MCH,RDW-SD,IgM,NLR and PLR)were selected by Lasso regression.(4)Logistic regression analysis showed that the four indexes of PLT,IgM,NLR and PLR were statistically significant(P < 0.05);MCH and RDW-SD had no statistical significance(P > 0.05).The PLT,NLR,PLR and IgM line graph prediction models were established,corresponding to the scores PLT(0-52.5 points),IgM(0-15points),NLR(0-66 points)and PLR(0-100 points).(5)the areas under ROC curves of the four indexes and their combined detection were respectively 0.75,0.837,0.886,0.634 and0.951,which had statistical significance(P>0.05).Conclusions: PLT,NLR,PLR and IgM are closely related to MP infection.The combined detection of the above four indicators improves the predictive efficacy of MP infection,and the evaluation efficacy of any one of the four indicators is lower than that of the combined detection indicators,indicating that the combined index detection has good value in clinical application and can provide a certain auxiliary reference for the early diagnosis of MP infection.
