Objective:To investigate the efficacy of compound betamisone in the treatment of rotator sleeve calcified tendinitis after shoulder arthroscopic calcification decompression or rotator cuff repair.Methods:Forty-four patients who were clearly diagnosed with calcific tendonitis of the rotator cuff and required arthroscopic surgery from January 2017 to December 2020 admitted to the Department of Sports Medicine of the First Affiliated Hospital of Xinjiang Medical University were selected for the study,and were divided into experimental and control groups according to the treatment plan,with 22 cases in each group.In the experimental group,there were 5 males and 17 females,aged 31-71 years,mean(52.64±11.03)years,13 cases of right shoulder and 9 cases of left shoulder;in the control group,there were 4 males and 18 females,aged 34-79 years,mean(54.00±10.45)years,14 cases of right shoulder and 8 cases of left shoulder.The control group was treated with arthroscopic calcification,acromiform decompression and or rotator cuff repair,while the experimental group was treated with compound betamisone injection into the joint cavity and subacromial injection on the basis of the treatment in the control group.The patient was hospitalized for 3-4 days and ice applied on the shoulder for 24-48 hours.At the same time,the patients were treated with detumescence and analgesia.There was no need for braking in the shoulder after operation.If rotator cuff repair was performed,the triangular towel or brace was fixed for 3 weeks,individualized functional rehabilitation training of affected limbs was carried out under the guidance of exercise rehabilitation physician,such as muscle strength training,passive pendulum exercise immediately after operation,active and passive exercise in the whole joint range on the second day after operation,etc.The pain visual analog scale score(VAS),shoulder motion degree and Constant-Murley shoulder function score were observed before operation,3days,1 month,3 months and 6 months after operation,and the therapeutic effects were compared between the two groups.Results All the subjects were followed up regularly,the effect of short-term treatment in the experimental group was significantly higher than that in the control group,and the pain score of VAS in the experimental group was significantly lower than that in the control group at 3 days and 1 month after operation(P< 0.05);The range of shoulder abduction in the experimental group was significantly better than that in the control group at 3 days and 1 month after operation,and the score of shoulder joint function in the experimental group was significantly better than that in the control group at 3 days,1 month and 3 months after operation(P < 0.05);No complications occurred in both groups.Conclusion Arthroscopic calcification and decompression,or rotator cuff repair can inhibit the inflammatory reaction by injecting compound betamisone into the joint cavity and subacromial injection in the short and early stage after arthroscopic calcification of rotator sleeve calcified tendon,reduce postoperative pain,improve the degree of shoulder stiffness,promote the repair and healing of injured rotator cuff tissue,early recovery of shoulder joint movement and function,so as to improve the therapeutic effect,safe and reliable. |
