Objective:To investigate the value of synthetic magnetic resonance imaging(SyMRI,Magnetic resonance imaging compilation,MAGiC)and its T1,T2 relaxation values and proton density(PD)values in the differential diagnosis of middle ear cholesteatoma and chronic suppurative otitis media.Method:The patients admitted to the Department of Otolaryngology,the First Affiliated Hospital of Xinjiang Medical University from June 2021 to December 2022 with suspected choleolatoma were collected.The patients were admitted as chief complaints of intermittent discharge of pus in both ears and progressive worsening of hearing loss.MRI multi-sequence scanning,including conventional MRI sequence and SyMRI sequence scanning,were performed before surgery.T1,T2 and PD were measured by two experienced physicians.The postoperative pathological results were used as the gold standard to compare the diagnostic efficacy of conventional MRI and SyMRI for middle ear cholesteatoma.To compare the difference of T1,T2 and PD values between middle ear cholesteatoma and chronic suppurative otitis media patients,and evaluate whether it has diagnostic significance.Results:The sensitivity,specificity,positive predictive value and negative predictive value of conventional MRI sequence and SyMRI sequence in the diagnosis of cholesteatoma were: 76.47%,85.71%,86.67%,75% and 82.35%,78.57%,82.35%,78.57%,respectively.There was no significant difference in diagnostic efficacy between conventional MRI sequence and SyMRI sequence by Fisher’s exact test.The T1 value of cholesteatoma was lower than that of chronic suppurative otitis media,and the T2 value was slightly lower than that of chronic suppurative otitis media,the difference was statistically significant.There was no significant difference in PD value between cholesteatoma and chronic suppurative otitis media(P >0.05).Conclusion:The synthetic MRI sequence has a good ability to distinguish cholesteroma of the middle ear from chronic suppurative otitis media,and there are significant differences in quantitative parameters T1 and T2 relaxation values,which has significant clinical value for the diagnosis of cholesteroma. |
