Objective:To evaluate the design and intervention effects of discharge planning information management program for stroke patients with dysphagia and caregivers based on nursing information system.Methods:The first part,based on evidence-based construction of stroke patients with dysphagia and caregivers discharge planning management program.With a framework of evidence-based continuous quality improvement,the rehabilitation process of stroke dysphagia was summarized and sorted out through evidence,and the discharge planning management for stroke patients and caregivers with dysphagia was constructed.Organize school and clinical nursing experts to hold expert meetings to revise the first draft of the stroke dysphagia discharge planning management scheme for patients and caregivers,and form its final version,as well as related processes and tools.The second part is to evaluate the clinic readiness of the discharge planning management program for stroke patients with dysphagia and caregivers.In April 2022,the General Information Questionnaire,the Clinic Readiness to Evidence-based Nursing Assessment scale and the Evidence-based Practice Questionnaire were used to survey nurses with practice place,to understand the current status of nurses’ readiness for upcoming evidence-based nursing practice activities and analyze the influencing factors.The third part is the construction and application of information management platform of discharge planning for stroke patients with dysphagia and caregivers.Based on the policy requirements of informationized hospital construction and so on,combined with the nursing information system to form an informationized discharge planning management mode,and improve the corresponding evaluation record form;Thirty-one cases of stroke patients with dysphagia and caregivers who met the inclusion criteria and were admitted to a general hospital in Bengbu City in September 2022(before the platform operation)were selected as the control group,and 32 cases admitted from October 2022 to December 2022(after the platform operation)were selected as the experimental group.Routine nursing in the Department of neurology was used in the control group.The experimental group applied the information management platform of discharge plan on this basis.Before and after the application of the platform,the self-management ability of the two groups of patients,the discharge readiness of patients and their caregivers,the rehabilitation of swallowing function and the satisfaction of nurses in the department with the platform were compared.Results:1.To construct discharge planning management for stroke patients with dysphagia and their caregivers.This study was based on evidence-based and research group research,and formed the first draft of discharge planning management for stroke patients with dysphagia and caregivers.Organization expert group meeting of school and clinical nurse specialists,for the construction of swallowing disorder in patients with cerebral apoplexy and caregivers to revise the first draft of the discharge planning management plan and the formation of the final version of swallowing disorder in patients with cerebral apoplexy and caregiver discharge planning management solutions,including 24~48 hours after admission to hospital,hospitalization,24 hours before discharge,on the day of discharge,discharge after 5 time node,It mainly includes 9 aspects,including screening,evaluation,planning,implementation,verification,referral,follow-up,team members and training,and includes relevant processes and tools.2.Clinic readiness to evidence-based nursing of clinical nurses.Among 166 nurses,the total score of clinic readiness to evidence-based nursing was(118.55±22.24),and the item mean was(3.82±0.72).The average score of each subscale item was from high to low:organizational environment subscale(3.96±0.78),evidence subscale(3.78±0.79),and promotion factor subscale(3.76±0.80).Multiple stepwise regression analysis showed that evidence-based practice attitude and the total score of evidence-based practice questionnaire were the main influencing factors of clinical nurses’ evidence-based practice readiness(P<0.05).3.Build and apply the informatization management platform of discharge planning for stroke patients with dysphagia and caregivers.Based on the nursing information system,with the app of 317 care and wechat to build an information management platform,including patient end and medical end;The results showed that the scores of patients’ self-management ability,patients’ and their caregivers’ readiness for discharge in the experimental group were higher than those in the control group,and the swallowing function rating was lower than that in the control group,with statistical significance(P<0.05).The satisfaction score of nurses on the platform was(92.92±21.74)points.Conclusions:1.Based on evidence-based construction,the discharge planning management scheme for stroke patients with dysphagia and their caregivers is scientific and can provide reference for clinical nursing work.2.The level of clinic readiness to evidence-based nursing of 166 nurses was moderate,which is conducive to clinical transformation of evidence.In the process of evidence-based practice,interventions should be implemented according to its influencing factors to improve the level of evidence-based practice readiness and promote the development of evidence-based practice.3.The application of the informatization management platform is helpful to improve patients’ self-management ability,improve the discharge readiness of patients and caregivers,promote the rehabilitation of stroke swallowing function,and improve patients’ health outcomes.Nurses have a high degree of satisfaction with this platform.
