Objective. This research was to study the needs of primary caregivers of strokepatient in different periods of family care, to analyze the dynamic change, to makeneed-oriented discharge planning intervention program, and to investigate the influence onneed satisfaction, self-efficacy and life quality of the caregivers.Methods. This research was performed in two consecutive phases. In the first phase,a descriptive research and convenient sampling method was used. Primary caregivers ofstroke patient from a hospital of Suzhou were selected as subjects. Caregiver DischargeNeeds Scale were used to measure the levels of needs and needs changing of primarycaregivers of stroke patient before discharge, one week after discharge and four week later.In the second phase, according to the need of caregivers, the study on the dischargeplanning intervention object to the primary caregivers of stroke patients was carried out.The subjects in the intervention group received need-oriented discharge planningintervention while those in the control group only received routine care. There were twoparts of the intervention given to the subjects in the intervention group. The subjectsreceived individual need support program when they were hospitalized and the telephoneintervention after discharge. The Caregiver Discharge Needs Satisfaction Scale, GeneralSelf-Efficacy Scale (GSES) and Quality of Life Scale were compared between two groupsone week after discharge and one month, three month later. Epidata3.1was used to enterdata. SPSS16.0was utilized to analyze the data.Results. The survey results of the primary caregivers’ needs in the first phase areas follows. Analysis of variance of repeated measure showed the needs were varied beforedischarge, one week after discharge and one month later(P<0.01).The needs of the caregivers before discharge are higher than discharged. The focus of the needs are variedin different time. After weighted, before discharge, Health Care Information Needs beforedischarge are the highest. In1week after discharge and4weeks later, Emotional SupportNeeds are the highest. The intervention investigation in the second phase shows as follows.①The total needs satisfaction of primary caregivers in the intervention group significantlyincreased compared with the control group(P<0.001).②Analysis of variance of repeatedmeasure showed the self-efficacy of primary caregivers in the intervention groupsignificantly enhanced compared with the control group(P<0.001).③The quality of life ofprimary caregivers in the intervention group improved compared with the controlgroup(P<0.05).Conclusions. The investigation indicates that the needs of primary caregiversbefore discharge are the greatest and the most urgent, especially the health careinformation needs. According to the results, the need-oriented discharge planning aredrawn up in the intervention stage. By satisfying the needs of primary caregivers, theself-efficacy is significantly enhanced and the quality of life is greatly improved. The resultshows that the need-oriented discharge planning intervention program is feasible and worthpopularizing.
