| Objective:Under the guidance of traditional Chinese medicine theory and modern medicine,the effect of the ratio of the inner diameter of medullary cavity to the diameter of Inter TAN in different planes on the hidden blood loss was analyzed.It is used to standardize the preoperative plan,select the most matching intramedullary nail according to the size of the medullary cavity,and reduce the hidden blood loss.According to the shortcomings of the existing intramedullary nail,it provides a basis for improving the design of the model and size of the intramedullary nail in the future.Methods:A total of 204 patients with femoral intertrochanteric fractures treated with Inter TAN nail in the Department of Orthopedics,the First Affiliated Hospital of Harbin Medical University from January 1,2016 to November 10,2021 were selected.Screening was performed according to inclusion and exclusion criteria.A total of 153 cases were screened.The ratio of the diameters of the three femoral medullary cavity planes to the diameter of the same plane of the intramedullary nail was measured.The hidden blood loss of the included cases was calculated and analyzed by IBM SPSS 26.0 software.The measurement data was expressed as(x ?±s).Fracture type,Inter TAN length,ratio of three femoral medullary cavity planes(Ra,Rb,Rc)were used as independent variables,and hidden blood loss was used as dependent variable.Multiple linear regression analysis was used to determine the related factors of hidden blood loss.The influence of the matching degree between the medullary cavity of the three planes and the implanted Inter TAN on the hidden blood loss was compared.Results:There was a linear relationship between hidden blood loss and fracture type,operation time,BMI,and the ratio of the diameter of the femoral medullary cavity to the diameter of Inter TAN at the beginning of the funnel of the intramedullary nail.The hidden blood loss of the high and medium fit groups was statistically significant compared with the hidden blood loss of the low fit group,but the hidden blood loss of the high fit group was not statistically significant compared with the hidden blood loss of the medium fit group.Conclusions:1.The higher the matching degree between Inter TAN and femoral marrow cavity,the less the hidden blood loss after intertrochanteric fracture.The hidden blood loss can be reduced when the matching degree reaches the middle group.Inter TAN,which can make the ratio of the femoral medullary cavity to the beginning of the intramedullary nail funnels less than 1.435,is recommended for the operation of intertrochanteric fractures,so as to reduce the hidden blood loss.2.Comminuted fracture,long operation time and high BMI will increase the hidden blood loss after intertrochanteric fracture surgery.3.Traditional Chinese medicine intervention can help to reduce the hidden blood loss after intertrochanteric fracture surgery. |
