| Objectives A retrospective study was conducted to compare the clinical efficacy of InterTAN and PFNA in AO/OTA-2018 31-A2 elderly patients with intertrochanteric fracture,providing reference for clinical selection.Methods Data of 175 elderly patients with AO/OTA-2018 31-A2 intertrochanteric fracture treated with InterTAN or PFNA in Tangshan Second Hospital from October 2016 to October 2020 were collected and summarized,and identified as the study objective,these patients were identified as the study target and screened strictly according to inclusion and exclusion criteria.Finally,78 patients meeting the requirements were obtained.According to different internal fixation methods,78 patients were divided into two groups,including InterTAN patients as Group A,PFNA patients as Group B.38 patients in Group A,male/female: 12/26;Average age: 73.29±7.73 years old.40 patients in Group B,male/female: 18/22;Average age: 75.65±8.94 years old.The length of surgical incision,operative time(operative time and intraoperative X-ray fluoroscopy time),perioperative visible blood loss(PVBL),perioperative hidden blood loss(PHBL),perioperative total blood loss(PTBL),postoperative first weight-bearing time,fracture healing time were measured,Harris score of hip joint,incidence of postoperative complications and other data were collected and sorted out.After statistical software analysis,the therapeutic effects of InterTAN and PFNA in the treatment of AO/OTA-2018 31-A2 elderly intertrochanteric fractures were compared.Results All 78 patients were fully followed up for 12 to 36 months.The PHBL of the two groups was 659.48±47.56 m L in Group A and 527.65±43.46 m L in Group B,and the difference between the two groups was statistically significant(P<0.05);There were statistically significant differences(P<0.05)between Group A and Group B in terms of operative time,intraoperative X-ray time,postoperative first weight-bearing time,average fracture healing time,the early postoperative Harris score(1,3,6 months postoperatively),and the incidence of postoperative complications(P<0.05).While in PVBL,surgical incision length,time from injury to surgery,average length of hospital stay,Harris score and the excellent and good rate of hip function at the late postoperative stage(12 months postoperatively),there were no significant differences(P>0.05).Conclusions For the elderly with AO/OTA-2018 31-A2 intertrochanteric fractures,InterTAN has better compression effect on the fracture end,earlier weight-bearing time,better postoperative hip function recovery,and higher early satisfaction,while PFNA has a potential risk of internal fixation failure PFNA surgery is more convenient,shorter operation time,less intraoperative blood loss and perioperative hidden blood loss.Figure19;Table9;Reference 83... |
