| ObjectiveThrough the traditional Chinese medicine inheritance calculation platform,this topic retrospectively excavates and collates the relevant data of the tutor’s treatment of DKD dyslipidemia medical records,and analyzes the medication characteristics of Yishen Qingxie Tongluo method in the treatment of DKD dyslipidemia.Through network pharmacology research,predict and explore the mechanism of drug action;to summarize the clinical experience of tutor in the diagnosis and treatment of abnormal lipid met abolism in DKD.Methods1.Basic information : 209 cases,134 males;female 75 cases;the age ranged from 35 to 80 years old,and 180 cases were over 50 years old.Statistics of TCM syndromes showed that there were 82 cases of spleen and kidney deficiency,damp turbidity and blood stasis type;liver and kidney yin deficiency,blood stasis block type 62 cases;lung and spleen qi deficiency,damp heat repression type 45 cases;yin and Yang deficiency,wet stasis block type 17 cases;the other 3 cases.2.Through the traditional Chinese medicine inheritance calculation platform(v3.0)related system,the basic information of patients and the data screening and entry of 209 first diagnosis prescriptions were completed,and the patient information,drug use frequency,drug nature,taste,meridian tropism,efficacy,core drugs,common drug combinations,drug core combinations and other related information were obtained.3.Core drug network pharmacology research,through TCMSP,HERB and other databases,screening the active ingredients and targets of core drugs;uniprot database was used to standardize the target proteins of drug active ingredients.Through Gene Cards,OMIM,Drugbank and other databases,the gene targets of diseases were searched.Venny software was used to obtain the intersection target of drugs and diseases.The intersection targets were imported into the STRING database for PPI network analysis.The PPI related data were imported into Cytoscape to screen the core targets.The intersection rake points were imported into the Metascape database to complete the GO and KEGG enrichment analysis,and the Metascape database was used to visualize the results of GO and KEGG enrichment analysis to explore the mechanism of core drugs in the treatment of DKD.Results1.Basic information : 209 cases,134 males,accounting for 65 %;75 women(35 %);the age distribution was between 35 and 80 years old,and 86 % were over 50 years old.The statistics of TCM syndromes showed that there were 82 cases of spleen and kidney deficiency and dampness and blood stasis type;liver and kidney yin deficiency,blood stasis block type 62 cases;lung and spleen qi deficiency,damp heat repression type 45 cases;yin and Yang deficiency,wet stasis block type 17 cases;the other 3 cases.2.Analysis of drug characteristics:209 cases were prescribed for the first time,involving 190 herbal medicines;among them,the top ten herbal medicines used in order of frequency were: astragalus membranaceus 200 times,chinese yam 195 times,lot us leaf 184 times,gynostemma 174 times,salviae miltiorrhizae173 times,rhizoma alismatis169 times,Leeches 165 times,dark plum164 times,corn stigma 161 times,andmotherwort herb 159 times;The results of the four natures of the drug show: flat 1584 times,cold1357 times,warm 1062 times,cool 74 times and hot 30 times;The results of the five flavors of the drug show: sweetness 2641 times,bitterness 1880 times,pungency 693 times,sourness 560 times and saltiness280 times;The results of the drug’s channel tropism show: liver meridian 2298 times,spleen meridian 2117 times,lung meridian 1438 times,kidney meridian 1360 times,etc;The drug efficacy results show that: 1264 times for tonifying deficiency,617 times for invigorating blood stasis,547 times for clearing heat,478 times for inducing diuresis,etc;Statistics of common drug combinations: e.g.astragalus membranaceus ~chinese yam,astragalus membranaceus ~ salviae miltiorrhizae,etc.;the core combination of drugs is 4 groups.3.Network pharmacology research results : 118 active ingredients of the top 10 core drugs,such as kaempferol,quercetin,luteolin,isorhamnetin,etc.There were 128 drug targets,2088 disease targets and 121 drug-disease intersection targets,among which the core targets were AKT1,STAT3,IL6,IL1 B,MAPK1,etc.There were 257 KEGG pathways,and the main signal pathways were lipid and atherosclerosis,HIF-1,AGE-RAGE,IL-17,TNF,etc.Conclusion1.The tutor believes that the core pathogenesis of DKD dyslipidemia is mostly based on the deficiency of spleen and kidney,and the accumulation of sugar,fat,phlegm and blood stasis is the standard of disease,which belongs to the syndrome of deficiency and excess,and the syndrome of deficiency and excess.And because of the machine legislation,the establishment of “Yishen Qingxie Tongluo ” law as its fundamental treatment;2.The instructor makes good use of Astragalus membranaceus,Chinese yam,lotus leaf,gynostemma,salvia miltiorrhizae,rhizoma alismatis,dark plum,corn stigma,to treat DKD with dyslipidemia as the main complication;he is also good at using combinations of drugs in pairs to reduce toxicity and increase effectiveness;3.Professor clinical use of sweet,flat products,emphasizing the kidney and spleen,pay attention to solid protection of the spleen and stomach,in order to strengthen the body;it is supplemented by bitter and cold products to clear damp heat,promote blood circulation,remove blood stasis and dredge collaterals,so that the evil has a way out,so as to improve many clinical symptoms,reduce urinary protein and delay the progress of renal failure.4.Network pharmacology research predicts the mechanism of action of the top 10 core drugs,which may play its anti-oxidative stress,anti-inflammatory,and immunomodulatory effects through its multi-component,multi-target,multi-channel,and systematic regulation,thereby delaying the occurrence of DKD and the progress of its
