Objective:Observe the distribution of TCM syndrome types in patients with stage CKD3-5(non-dialysis)with MS,analyze the association between TCM syndrome types and general clinical data and laboratory examination indicators, and provide some reference for TCM syndrome differentiation treatment in stage CKD 3-5with metabolic syndrome.Method:MS patients with CKD stage 3-5(non-dialysis)hospitalized from December2021 to December 2022 at the First Affiliated Hospital of Heilongjiang University of Chinese Medicine in December 2022 were collected, and 190 cases were selected according to the inclusion and exclusion criteria. The raw data was included in the excel data table including:the general patient data(name, gender, age, medical record number, blood pressure, abdominal circumference, common comorbidities, primary disease,CKD stage),relevant laboratory examination indicators(TG,TC,FBG,HDL-C,Scr,SUA),TCM syndrome differentiation and classification. The statistical software spss26.0 was used to process and analyze data, composition ratio or rate to analyze general data;chi-square test(x~2test)or one-way analysis of variance(One-Way Anova)and Spearman correlation between diagnostic index of metabolic syndrome and risk factors.Result:1.Among the 190 CKD stage 3-5(nondialysis)combined MS patients included,117(61.6%)were male and 73(38.4%)were female, with a mean age of55.42±12.28 years. The primary disease distribution was the majority of chronic glomerulonephritis(38.4%),followed by hypertensive nephropathy(32.6%)and diabetic nephropathy(28.9%).2.The frequencies of TCM deficiency in MS patients with CKD stage 3-5(non-dialysis)were spleen and kidney Qi deficiency(36.3%),spleen and kidney Yang deficiency(30.5%),Yin and Yang deficiency(15.3%),liver and kidney Yin deficiency(8.9%),qi and Yin deficiency(8.9%);the frequency of wet turbidity(59.5%),damp heat(24.2%),blood stasis(12.1%),and water and qi(4.2%).3.The distribution of spleen and kidney qi deficiency and dampness-heat syndrome in MS patients with CKD 3-5(non-dialysis)decreased with the progression of CKD stage 3-5(p<0.05),and the distribution of Yin and Yang deficiency and wet turbidity increased with the progression of CKD stage 3-5(p<0.05).4.Blood stasis syndrome in CKD stage 3-5(non-dialysis)MS(p<0.05);spleen and kidney qi deficiency was the highest in patients aged 41-59 years, and the distribution of Yin and Yang deficiency increased with age(p<0.05);the distribution of liver and kidney Yin deficiency gradually increased with increasing blood pressure grade(p<0.05).5.In CKD stage 3-5(non-dialysis)with MS patients, spleen and kidney qi deficiency accounted for the highest proportion in hypertensive nephropathy(40.3%),and spleen and kidney Yang deficiency accounted for the highest proportion in chronic glomerulonephritis(38.4%).6.In patients with CKD 3-5(non-dialysis),the average level of HDL-C,the average level of Scr was the highest(p<0.05),and the average level of Scr was higher in the standard evidence(p<0.05).7.Alcohol consumption was positively correlated with FBG,TG,and abdominal circumference and negatively with HDL-C,and its typical correlation coefficient was 0.165,0.275,0.275,0.270-0-173,indicating drinking with FBG,TG,abdominal circumference, and a negative correlation with HDL-C.Smoking was positively associated with TC,and its canonical correlation coefficient was0.221,indicating a positive relationship between smoking and TC.Aging was negatively correlated with abdominal circumference, and its typical correlation coefficient was-0.236,indicating that there was a negative correlation between aging and abdominal circumference.Conclusion:1.Chronic glomerulonephritis is the most common disease in the primary disease of CKD stage 3-5(non-dialysis)patients with MS.2.In patients with CKD stage 3-5(non-dialysis)combined with MS,the deficiency syndrome is spleen and kidney gas deficiency, and the empirical evidence is wet turbidity syndrome is most common.3.In CKD stage 3-5(non-dialysis)and MS patients with CKD progression, the distribution of spleen and kidney gas deficiency syndrome and damp-heat syndrome gradually decreased, and the distribution of Yin and Yang deficiency syndrome and wet turbidity syndrome gradually increased.4.The distribution of blood stasis syndrome in CKD stage 3-5(non-dialysis)MS patients was significantly higher in men than women;spleen and kidney qi deficiency syndrome was the highest proportion in patients aged 41-59 years;the distribution of Yin and Yang deficiency syndrome gradually increased with age;and the distribution of liver and kidney Yin deficiency syndrome gradually increased with increasing blood pressure grade.5.CKD stage 3-5(non-dialysis)MS patients have the highest proportion of spleen and kidney qi deficiency and the highest proportion in chronic glomerulonephritis.6.In MS patients of CKD stage 3-5(non-dialysis),the average level of HDL-C of Yin and Yin and the average level of Yin and Yang were the highest in this deficiency syndrome, and the average level of scr of water and gas was relatively high in the standard evidence.7.Alcohol consumption, smoking and aging are risk factors for CKD stage3-5(non-dialysis)patients with MS.
