TCM Constitution Of Patients With Stage CKD1-4 Complicated With Hypertention And Analysis Of Related Factors

ObjectiveThis project collects medical information of CKD1-4 patients with concomitant hypertension,summarizes the distribution of traditional Chinese medicine constitution,analyzes disease related factors and susceptible constitution;Explore the relevant risk factors of disease susceptibility constitution,in order to provide direction for personalized protection.MethodsThis study selected 270 patients with CKD1-4 concomitant hypertension who visited the nephrology department and ward of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from December 2020 to December 2022(including 88 patients with CKD1-2concomitant hypertension,91 patients with CKD3 concomitant hypertension,and 91 patients with CKD4 concomitant hypertension),and collected data through a survey questionnaire;Summarize and organize the patient’s general situation,laboratory indicators,and traditional Chinese medicine constitution data into Microsoft Excel,and then establish a database using SPSS27.0software for statistical analysis.Results1.In this study,there were significant differences in age,dietary preferences,SBP control level,DBP control level,CKD course,hyperlipidemia course,and levels of Scr,urea nitrogen,and hemoglobin among CKD1-2,CKD3,and CKD4 patients with hypertension.There were also significant differences(p<0.05)in gender,marital status,occupational nature,monthly family income,BMI,exercise status,smoking history,drinking history There was no significant difference in the history of hyperuricemia and the levels of total cholesterol,triglyceride,lipoprotein a,low-density lipoprotein,blood uric acid and serum albumin(p>0.05).2.In this study,the distribution of traditional Chinese medicine constitution in CKD1-4 stage with hypertension was 58 cases of Qi deficiency,52 cases of Yang deficiency,16 cases of Yin deficiency,17 cases of Ping He quality,30 cases of phlegm dampness quality,27 cases of damp heat quality,51 cases of blood stasis quality,and 14 cases of Qi stagnation quality.There was a significant difference in traditional Chinese medicine constitution between CKD1-2 stage with hypertension,CKD3 stage with hypertension,and CKD4 stage with hypertension(p<0.05),among which Qi deficiency quality,Yang deficiency quality,blood stasis quality,and phlegm dampness quality accounted for the majority,And there is a significant difference in distribution(p<0.05),indicating a disease susceptibility constitution.3.In this study,there were significant differences between CKD1-2combined with hypertension susceptibility constitution and dietary preferences,age,CKD course,urea nitrogen,Scr,and hemoglobin(p<0.05),CKD3 combined with hypertension susceptibility constitution and CKD course and urea nitrogen(p<0.05),and CKD4 combined with hypertension susceptibility constitution and dietary preferences(p<0.05).There is a positive correlation between CKD1-2 stage with hypertension and qi deficiency and age(OR=1.079,p=0.000);Yang deficiency is positively correlated with dietary preference(OR=2.134,p=0.026)and urea nitrogen(OR=1.434,p=0.039),while negatively correlated with hemoglobin(OR=0.951,p=0.005);The quality of blood stasis is positively correlated with the course of CKD(OR=2.101,p=0.017),and negatively correlated with hemoglobin(OR=0.949,p=0.013);There is a positive correlation between phlegm dampness and dietary preference(OR=2.241,p=0.043);There is a positive correlation between CKD3 stage complicated with hypertension yang deficiency and urea nitrogen(OR=1.309,p=0.013).Conclusions1.In this study,patients with CKD1-2 stage concomitant hypertension,CKD3 stage concomitant hypertension,and CKD4 stage concomitant hypertension showed intergroup differences in age,dietary preferences,SBP and DBP control levels,CKD course,hyperlipidemia course,and Scr,urea nitrogen,and hemoglobin levels.2.In this study,the number of traditional Chinese medicine constitution in CKD1-4 patients with hypertension was ranked from high to low as follows: Qi deficiency constitution,Yang deficiency constitution,blood stasis constitution,phlegm dampness constitution,damp heat constitution,peace constitution,Yin deficiency constitution,Qi stagnation constitution,and innate constitution.The susceptible constitution was Qi deficiency constitution,Yang deficiency constitution,blood stasis constitution,and phlegm dampness constitution.3.In this study,the relevant factors of CKD1-2 stage combined with hypertension susceptibility constitution are dietary preference,age,CKD course,urea nitrogen,Scr,and hemoglobin.Among them,age is a risk factor for qi deficiency constitution,dietary preference is a risk factor for yang deficiency constitution,blood stasis constitution,and phlegm dampness constitution,urea nitrogen is a risk factor for yang deficiency constitution,CKD course is a risk factor for blood stasis constitution,and hemoglobin is a protective factor for yang deficiency constitution and blood stasis constitution;The related factors of CKD3 combined with hypertension susceptibility constitution are the course of CKD and urea nitrogen,among which urea nitrogen is a risk factor for yang deficiency;The related factor of CKD4 combined with hypertension susceptibility constitution is dietary preference.
