Exploring The Difference Between Blood Stasis Evidence And Non-blood Stasis Evidence In HFmrEF Based On Echocardiographic Features

Objective: To analyze the differences in echocardiographic indices between patients with and without blood stasis in heart failure with mid-range ejection fraction(HFmrEF),and to analyze the association between echocardiographic indices and blood stasis,so as to provide objective indices for the identification of blood stasis in HFmrEF.The differences of echocardiographic indices between different blood stasis evidence of HFmrEF were also analyzed to improve the understanding of different blood stasis evidence of HFmrEF,to improve the clinical traditional Chinese medicine diagnosis and to further improve the clinical efficacy of traditional Chinese medicine.Methods: One hundred and forty patients with a diagnosis of HFmrEF who attended the outpatient clinic and ward of the Department of Cardiovascular Diseases of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from November 2021 to February 2023 were collected.Basic information,medical histories,information on the four Chinese medicine consultations,and echocardiography reports of the enrolled patients were recorded,and the enrolled patients were evaluated for cardiac function classification.SPSS 26.0 was used to statistically analyze the recorded data,and binary Logistic regression analysis was used to explore the correlation between HFmrEF blood stasis evidence and echocardiographic indices.To explore the association between echocardiographic indices and HFmrEF blood stasis evidence.Results:1.Patients with HFmrEF were over 60 years old,and the gender ratio was predominantly male.There was a difference in the smoking rate between patients with HFmrEF with and without blood stasis,and the smoking rate in patients with blood stasis was greater than that in patients without blood stasis(P<0.05).2.The proportion of HFmrEF patients with previous coronary artery disease,myocardial infarction,and previous coronary intervention was greater than that of patients without blood stasis(P<0.05).3.LVIDd,LVIDs,IVSTd,and LVMI were greater in patients with HFmrEF with blood stasis than in patients without blood stasis(P<0.05).The binary Logistic regression analysis showed that LVIDd,LVIDs and IVSTd were risk factors for the development of HFmrEF with blood stasis(P<0.05).4.The E peak,E/A,and E' values of HFmrEF patients with blood stasis evidence were smaller than those of patients without blood stasis evidence,while E/E' values were larger than those of patients without blood stasis evidence(P<0.05).Binary Logistic regression analysis showed that E peak and E/A value were protective factors for the occurrence of HFmrEF blood stasis evidence(P<0.05).5.LVIDd and LVMI of patients in the HFmrEF Yang deficiency blood stasis group were significantly greater than those in the Qi deficiency blood stasis group and the Qi-Yin two deficiency blood stasis group,while S' was smaller than those in the Qi deficiency blood stasis group and the Qi-Yin two deficiency blood stasis group(P<0.05).6.There was no significant difference in cardiac function classification between HFmrEF blood stasis evidence and non-blood stasis evidence and between different blood stasis evidence of HFmrEF(P>0.05).Conclusion:1.LVIDd,LVIDs,IVSTd,E peak,and E/A values are echocardiographic indices that correlate with HFmrEF blood stasis evidence and can be used as objective indices for the identification of HFmrEF blood stasis evidence.2.The LVMI and LVIDd of HFmrEF Yang deficiency blood stasis evidence were greater than Qi deficiency blood stasis evidence and Qi-Yin two deficiency blood stasis evidence,and S' was less than Qi deficiency blood stasis evidence and Qi-Yin two deficiency blood stasis evidence.3.There was no significant difference in cardiac function classifi cation between HFmrEF blood stasis evidence and non-blood stasis evidence,and between different blood stasis evidence of HFmrEF.
