| Objective: By comparing the levels of N-terminal pro-brain natriuretic peptide(NT-proBNP)in patients with chronic heart failure complicated with periodontitis and patients with chronic heart failure without periodontitis,the relationship between plasma and saliva NT-proBNP levels in all subjects was analyzed,and the relationship between chronic heart failure and periodontitis was discussed,and the significance of saliva NT-proBNP detection in patients with chronic heart failure was analyzed.Methods: From December 2021 to April 2022,ninety-six inpatients with chronic heart failure diagnosed by the Department of Cardiovascular Medicine and General Medicine of Chaohu Hospital affiliated to Anhui Medical University were selected as the study subjects.They were divided into the case group(forty-eight cases)and the control group(forty-eight cases)according to whether they were associated with periodontitis.The New York Heart Association(NYHA)classification was used for cardiac function classification of all subjects.Both groups were Record general information and periodontal parameters:attachment loss(AL),probing depth(PD),bleeding on probing(BOP),and calculus index(CI),used the Oral Health Impact Scale(OHIP-14)to score the oral health,and measured the NT-proBNP levels in plasma and saliva at admission and discharge.The influence of the above factors on NT-proBNP and the consistency and correlation of plasma-saliva NT-proBNP were analyzed.Results: There was no significant difference in sex,age,blood pressure,BMI,hypertension history,diabetes history,smoking,and drinking history between the case group and the control group(P>0.05),but blood lipid had significant differences(P<0.05).The levels of periodontal parameters AL,PD,BOP,CI,NYHA classification and the scores of OHIP-14 in the case group were higher than those in the control group,with statistically significant differences(P<0.05).The level of NT-proBNP in admission was higher than that in discharge,with statistically significant differences(P<0.05).The levels of plasma NT-Pro BNP concentrations and the percentage of decrease at admission and discharge in the case group were higher than those in the control group,with statistically significant differences(P<0.05).There was no significant difference in the concentration of NT-Pro BNP in saliva and the percentage of decrease at admission and discharge between the two groups(P>0.05).The bland-altman consistency analysis showed that the consistency of NT-proBNP in plasma and saliva was good.The results of spearman correlation analysis showed that NT-proBNP in plasma and saliva of the two groups showed weak correlation(the case group:rs=0.312,P=0.002),the control group:rs=0.360,P=0.000).Conclusion:(1)There is a correlation between periodontitis and chronic heart failure,and periodontitis can affect the levels of NT-proBNP and cardiac function grading in patients with chronic heart failure.(2)Saliva detection of NT-proBNP may also provide a new idea for the auxiliary detection of chronic heart failure. |
