The Guidance Value And Prognostic Influence Of BNP In Patients With Chronic Heart Failure

Objectives:To explore the guiding value and the influence on the prognosis of plasma brain natriuretic peptide (BNP) in the treatment of patients with chronic heart failure(CHF).Methods:Selected70patients with chronic heart failure, randomly divided into the BNP steering group and Conventional treatment steering group. Two groups are specified treated by the same doctor. All patients were followed up for9months, and the BNP level,6minutes walk test,the Minnesota living with heart failure questionnaire testing would be tested at just admission,1months,3months,6months,9months. But in the BNP steering group, the doctor treated patients according to the BNP level combined with clinical symptoms,in Conventional treatment steering group, the doctor treated patients according to their experiences combined with clinical symptoms symptoms.The main evaluation indicators were mortality rate, readmission rate and BNP level. The secondary evaluation indicators were6minutes walk test and the Minnesota living with heart failure questionnaire. All datas were gived statistical analysis by SPSS17.0software,the aim is to analyse the differences of the main ending events, secondary ending events and drug dose’s adjustment between BNP steering group and Conventional treatment steering group.Results:(1) There is no statistical significance of the main evaluation indicators including mortality and readmission rate between the BNP steering group and Conventional treatment steering group(P>0.05).(2) There is no statistical significance of secondary evaluation indicators including6minutes walk test and the Minnesota living with heart failure questionnaire between BNP steering group and Conventional treatment steering group (P>0.05).(3) There is no obvious difference between BNP steering group and Conventional treatment steering group in doses of aldosterone receptor antagonist and diuretic (P>0.05).(4) Compare with Conventional treatment steering group, the1/2target dose rate of ACEI drugs and beta blockers increased significantly in the BNP steering group (P<0.05).(5) Compare with Conventional treatment steering group, the dosage of ACEI drugs and beta blockers increased significantly in the BNP steering group (P<0.01).Conclusion:The monitoring of BNP level can guide treatments of patients wit h chronic heart failure, but that can not improve patients’prognosis and life qu ality.
