Objective: We survey the serum albumin level,microinflammatory state and quality of life scores of patients with gastric cancer chemotherapy and explore the effect of serum albumin on microinflammation and quality of life to provide a timely diagnosis and treatment of nutritional status and inflammatory response with them in the future,to improve the quality of life of patients based on the research.Methods: A cross-sectional survey was conducted in the First Affiliated Hospital of University of Science and Technology of China from November 2021 to August 2022.230 patients were effectively collected finally,who were first admitted to the hospital for gastric cancer chemotherapy selected as the subjects of this study.Face-to-face surveys were conducted by trained investigators through questionnaires.The survey included: patient demographic information,clinicopathological data,laboratory nutrition and inflammation data;CRP(c-reactive protein,CRP)>3 was defined as the occurrence of microinflammation,and the QLICP-ST(quality of life Instruments for cancer PatientsStomach cancer,QLICP-ST)scale was used to collect the scores of each dimension of patients ’ quality of life in the study.According to the serum albumin level,divided into low protein group and normal group.The measurement data conforming to the normal distribution were described by mean standard deviation,otherwise,described by median and quartile spacing.Count data adoption rate or composition ratio were described.T test or 2 test or rank sum test and Mann Whitney U test were used to compare the differences between groups.Binary Logistic regression model was used to analyze the effect of serum albumin on microinflammation,and multiple linear regression model was used to analyze the effect of serum albumin on quality of life.Result: Total of 230 patients were finally effectively collected according to the inclusion and exclusion criteria.The average BMI of patients ranged from 15.58 to 34.94,with a minimum age of 32 and a maximum age of 84 years.Statistical differences in age,occupation,combined basic diseases,tumor stage and lymphatic metastasis between the normal and low protein group(P<0.05).There were significant differences on the distribution of prealbumin,hemoglobin,retinol binding protein,albumin and LMR(lymphocyte-to-monocyte ratio,LMR)in the two groups(P<0.05).There were also statistically significant in the scores of microinflammation and quality of life between the two groups(P<0.05).After adjusting for demographic characteristics(gender,age,occupation,etc.)and clinicopathological data(surgical method,lymph node metastasis,tumor location,etc.),patients who had a higher risk of microinflammation(OR=3.666,95% CI=1.558-8.626)in the low protein group compared with the normal group.Further stratified analysis according to gender.Compared with the normal group,patients who had a higher risk of microinflammation(OR=4.416,95%CI=1.692-12.173)in the low protein group among male patients.The risk of microinflammation was only 1.687 folds in the low protein group,whereas the difference was not statistically significant(OR=1.687,95%CI=0.246-11.562)among female patients.The results of multivariate linear regression analysis indicated that patients had lower scores on social function and specific modules in the low protein group compared with the normal group(standardized(SD)β=-2.883,SD β=-3.170).Further stratified analysis according to gender.Compared with the normal group,who only had lower scores in the specific module in the low protein group among male patients(SD β=-3.626),the physical function score and social function score of the low protein group were lower among female patients(SD =-2.124,SD β=-2.647),and the difference was statistically significant(P<0.05).No statistically significant in male and female patients on the scores of rest dimensions.Conclusion: The results of this study indicated that low protein levels had a higher risk of microinflammation,especially male patients with gastric cancer before chemotherapy had a higher risk of microinflammation.additionally,the quality of life of patients was at a poor level,particularly poor social function and more specific symptoms in the low protein group.
