Objective In this study,we sought to identify lipid parameters that predict the risk of acute ischemic stroke by evaluating nontraditional lipid indexes in patients with acute ischemic stroke(AIS),and to provide an important basis for improving the diagnosis and longterm quality of life of stroke patients,thereby further guiding the clinical treatment and long-term outcomes of stroke patients.Methods A total of 424 patients with acute ischemic stroke admitted to the Department of Neurology,Suzhou Hospital Affiliated to Anhui Medical University from December 2020to December 2022 were collected as the case group,while 202 patients with non cerebral infarction in our hospital were selected as the control group.General data,laboratory parameters,etc.,were recorded for both groups;Non traditional lipid parameters were calculated from serum levels of total cholesterol(TC),triglycerides(TG),low density lipoprotein cholesterol(LDL-C)and high density lipoprotein cholesterol(HDL-C),including non-high density lipoprotein cholesterol(non HDLC)Residual cholesterol(RC),triglyceride glucose index(TYG),TC / HDL-C,TG / HDL-C and LDL-C / HDLC,RC / HDL-C.Binary logistic regression analysis and receiver operating characteristic curve(ROC)were used to evaluate the relationship between nontraditional lipid parameters and AIS.Results1.A total of 424 patients with acute ischemic stroke were included in the study,including261 females and 163 males,with a mean age of 67.45 ± 12.82 years.A total of 202 healthy individuals were included in the healthy control group,including 124 females and 78 males,with a mean age of 65.35 ± 13.63 years.Gender age was not statistically significant between the two groups.2.Systolic blood pressure,diastolic blood pressure,blood glucose,blood homocysteine,creatinine,uric acid,white blood cells,pulse pressure index,arteriosclerosis index,D dimer,and fibrinogen were higher in the case group than in the control group,and the differences were statistically significant(P < 0.05).Platelets were lower in the case group than in the control group,with statistically significant differences(P < 0.05).There were no significant differences in gender,age,BMI,alt,alt,RBC,Hb between the two groups(P > 0.05).3.The levels of TC,TG,LDL-C,TC / HDL-C,and RC and the levels of TG / HDL-C,TYG,and RC / HDL-C in the case group were higher than those in the control group,with statistically significant differences(P < 0.05).The level of HDL-C in the case group was lower than that in the control group,and the difference was statistically significant(P < 0.05).However,there was no statistical significance between the two groups in LDLC / HDL-C,non HDLC(P > 0.05).4.Logistic regression analysis of the TC(OR=1.81,95%CI=1.06-3.94)、LDL-C(OR=0.26,95%CI=0.14-0.49)、HDL-C(OR=0.04,95%CI=0.01-0.10)、TC/HDLC(OR=1.52,95%CI=1.10-2.08)、RC(OR=1.75,95%CI=1.17-2.34)、Non-HDLc(OR=0.43,95%CI=0.23-0.80)、TG/HDL-C(OR=0.45,95%CI=0.31-0.66)、Ty G(OR=2.63,95%CI=1.57-4.41)、RC/HDL-C(OR=5.54,95%CI=3.16-9.71),,Is an independent influencing factor of acute ischemic stroke.5.(P < 0.05)for the TC,TG,LDL-C,HDL-C,RC / TC / HDL-C,TG / HDL-C,TG / HDLC,and TG,and RC / HDL-C,vs 0.440,0.568,0.568,0.410,0.357,0.640,0.597,0.619,0.591,0.591,0.677,non-HDL-C,non-HDL-C,non HDL-C,and non HDL-C,P < 0.510,0.480,non the predictive ability of nontraditional lipid indicators was stronger than that of conventional lipids.6.A positive correlation(P < 0.05)was found between non-traditional lipid parameters and clinical indexes uric acid,platelet and blood glucose in AIS patients,and the AUC results of combining non-traditional lipids and these indexes showed that the AUC of combined indexes was greater than single non-traditional lipid parameters,i.e.,the combined indexes had stronger predictive efficacy for AIS than single non-traditional lipid indexes,among which RC / HDL-C combined with uric acid had the strongest predictive efficacy.Conclusion1.The four traditional blood lipids,TG / HDL-C,TC / HDL-C and RC / HDL-C,RC,and TYG were associated with the incidence of acute ischemic stroke.2.The four terms traditional lipid profile,LDL / HDL-C,TG / HDL-C,TC / HDL-C and RC / HDL-C,RC,TYG,non-HDL-C were independent risk factors for AIS in addition to LDL-C / HDL-C.3.The predictive power of non-traditional lipid parameters was stronger than that of traditional lipid parameters in predicting AIS,with the strongest predictive power being for lipid ratio RC / HDL-C.4.Non traditional lipid parameters in AIS patients there were correlations with clinical indexes uric acid,platelets,blood glucose,and the AUC results of combining nontraditional lipids with these indexes showed that the combined indexes had stronger predictive efficacy for AIS than single non-traditional lipid indexes.5.Incorporation of these indicators into routine clinical settings may help to identify highrisk individuals for stroke patients and guide effective treatment modalities.
