Aims:This study investigated the prevalence of subjective cognitive decline(SCD)among elderly people aged 60 and above in the community of Yongqiao District,Suzhou City,Anhui Province,explored the influencing factors of SCD and the relationship between blood lipid level and SCD in the elderly people in the community,identified,diagnosed and intervened in the occurrence and development of SCD early,and delayed its progression to Alzheimer’s disease(AD).Methods:In this study,603 people aged≥60 years old were selected from Street Community,Suzhou City,Anhui Province by the convenient sampling method in December 2020.They were grouped by the subjective cognitive questionnaire(SCD-Q9),with a score of<5 points as the normal control group and a score of≥5 points as the SCD group.At the same time,the general information of the subjects,such as name,sex,age,height/weight(BMI),education level,annual family income,hypertension history,diabetes history,myocardial infarction history,total cholesterol(TC),triglyceride(TG),low-density lipoprotein(LDL)and high-density lipoprotein(HDL)values,as well as the converted non-traditional lipid indicators TG/HDL ratio,TC/HDL ratio,LDL/HDL ratio,and non high-density lipoprotein(Non HDL),Single factor analysis method was used to compare the two groups of data to analyze the general situation of the elderly in the community and whether there is any correlation between the blood lipid indicators and SCD.Multivariate logistic regression model was used to analyze the relevant influencing factors of patients with SCD.Results:(1)A total of 603 people were included in the study,including 262(43.4%)males and 341(56.6%)females.The age range of the included population was 60-76 years old,with an average age of 67.63 ± 4.35 years old.According to statistics,the total number of people in the SCD group was 395(65.5%),the score of the SCD-Q9 scale was 6.01±0.02,the total number of people in the normal control group was 208(34.5%),and the score of the SCD-Q9 scale was 1.48 ± 0.13.(2)The univariate analysis showed that there was a statistically significant difference between the two groups in terms of age,gender,smoking,drinking,education,and annual family income(all P<0.05).There was no statistically significant difference between the two groups in terms of physical exercise,regular diet,sleep disorders,hypertension history,myocardial infarction history,diabetes history,and body mass index(all P>0.05).(3)The analysis of each blood lipid index between the two groups showed that the difference between HDL,TC/HDL ratio and LDL/HDL ratio was statistically significant(all P < 0.05),while the difference between TG,TC,TG/HDL ratio and No-HDL between the two groups was not statistically significant(all P>0.05).(4)Logistic regression analysis of the two groups showed that sex(OR=6.188,95% CI: 3.977-9.627,P < 0.001),age(OR=1.412,95% CI: 1.133-1.760,P=0.002),smoking(OR=3.846,95% CI: 1.882-7.856,P<0.001),alcohol consumption(OR=2.936,95% CI: 1.561-5.525,P=0.001),and TC/HDL ratio(OR=2.369,95% CI: 1.376-4.171,P=0.002)were the influencing factors of SCD in the elderly population in the community,LDL/HDL ratio(OR=0.561,95% CI: 0.323-0.976,P=0.004)is a protective factor for SCD in the elderly population in the community.(5)Logistic regression analysis showed that after adjusting for age,sex,smoking history,drinking history,education level and family annual income,among the non-traditional lipid indicators,TC/HDL(OR=3.398,95% CI: 1.999-5.776,P<0.001),TG/HDL(OR=0.467,95% CI: 0.176-0.790,P=0.005)were still correlated with SCD.(6)The sensitivity of the TC/HDL ratio was higher than that of HDL,which had certain reference significance for the diagnostic evaluation of early SCD.Conclusion:(1)Among the elderly population in the community,the prevalence of SCD is65.5%,which is influenced by gender,age,smoking,alcohol consumption,education level,family income,HDL,TC/HDL ratio,and LDL/HDL ratio;(2)Compared with the normal control group,the lower the HDL,the higher the TC/HDL ratio,the higher the score of subjective cognitive decline,the more serious the cognitive impairment,and the higher the diagnostic value of TC/HDL ratio for SCD;(3)On the whole,non-traditional lipid indicators can provide another relevant diagnostic thinking than traditional lipid indicators,and have a clearer significance in this study.When conducting health screening for the elderly population in the community,attention should be paid to the risk factors related to SCD,early diagnosis should be made to the SCD population,and reasonable intervention should be taken to delay and prevent its development into AD.
