Objective To investigate the patterns and characteristics of tinnitus and insomnia symptoms in adult patients with sensorineural deafness before and after cochlear implantation and the correlation between tinnitus and insomnia symptoms.Methods In this study,89 adult patients who underwent cochlear implantation in our department from January 2020 to December 2021 were used as study subjects,and all patients independently completed the Tinnitus Disability Inventory(THI)and Insomnia Severity Index(ISI)before and 6 months after surgery,respectively,to investigate the characteristics of changes in tinnitus and insomnia symptoms before and after cochlear implantation and to investigate the relationship between tinnitus and insomnia symptoms.Results Tinnitus symptoms were significantly reduced after cochlear implantation compared with the preoperative period,and the postoperative THI scores were significantly lower than the preoperative period(t=-11.908,P<0.05),and the results of different subgroups were similar to the overall;insomnia symptoms were also significantly improved after cochlear implantation compared with the preoperative period,and the postoperative ISI scores were significantly lower than the preoperative period(Z=-6.290,P<0.05).Further multivariate analysis revealed that residual hearing,pre-implantation THI score,pre-implantation ISI score and THI score changes were still closely related to insomnia improvement.In different logistic regression models,THI score changes was positively associated with insomnia improvement in the model without adjustment for covariates,OR=1.12,95%confidence interval 1.06-1.18.model 1 adjusted the age and gender,OR=1.13,95%confidence interval 1.07-1.19.model 2 adjusted the age,gender,residual hearing,pre-implantation ISI score and pre-implantation THI score,OR=1.19,95% confidence interval 1.06-1.35.Logistic regression subgroup analysis was performed according to age,gender and residual hearing,and no statistical difference was found between the groups(P>0.05).Correlation analysis revealed a positive correlation between THI scores and ISI scores before cochlear implantation(r=0.85,P<0.05),and a positive correlation between the changes of THI scores and the changes of ISI scores after cochlear implantation(r=0.79,P<0.05).Conclusion 1.Cochlear implantation has good therapeutic effects in patients with severe sensorineural deafness with tinnitus and insomnia;after cochlear implantation,both tinnitus and insomnia are relieved and improved to different degrees,and the effects are positive.2.Before and after cochlear implantation,the severity of tinnitus is closely related to the severity of insomnia;after cochlear implantation,insomnia and tinnitus are similarly improved.3.Cochlear implantation can not only improve the auditory perception of adult patients with severe tinnitus and insomnia,but also improve the symptoms of tinnitus and insomnia.4.It is recommended that cochlear implantation should be more actively implemented and promoted in clinical practice for patients with severe sensorineural deafness with tinnitus and insomnia.
