Objective To explore the application effect of family participatory nursing based on cloud platform on self-management behavior and quality of life of epilepsy patients undergoing vagal nerve stimulation.It provides a certain basis for improving the health education effect of clinical medical staff,improving the disease cognition level of VNS epilepsy patients,and improving their self-management behavior and quality of life.Methods A random number table was used to divide 84 epileptic patients who performed with vagus nerve stimulation treatment from a department of neurosurgery of a third-class A hospital in Anhui Province from January 2021 to October 2022 into two groups: cloud platform family participation nursing group(observation group)and routine health education group(control group),each with 42 patients.The control group carried out routine nursing and follow-up methods,while the observation group carried out a cloud platform based family participatory nursing plan on the basis of routine nursing,including the establishment of a cloud follow-up team,intelligent promotion of health education content on the platform,online consultation,family training,family communication meetings,etc.The intervention time was during hospitalization and 3months after discharge.The patients in the two groups were evaluated with the following items: Knowledge Awareness Questionnaire of Epilepsy Patients,the Hospital Anxiety and Depression Scale(HADS),the Epilepsy Self-Management Behavior Scale(ESMS),the Quality Of Life In Epilepsy-31(QOLIE-31)and the postoperative complications.Results A total of 8 cases were lost during the intervention,and the data collection of76 patients with VNS epilepsy was finally completed,including 37 cases in the control group and 39 cases in the observation group.(1)Comparison of the awareness of epilepsy knowledge: There was a significant difference in epilepsy knowledge awareness between the two groups of patients 3 months after discharge(P<0.05);There was a significant difference in the level of awareness of epilepsy knowledge among the families of the two groups of patients 1month and 3 months after discharge(P<0.05).(2)Comparison of the scores of the Hospital Anxiety and Depression Scale:Compared between groups,the observation group showed a significant decrease in HADS and HADS-D scores compared to the control group one month after discharge(P<0.05);After 3 months of discharge,the scores of HADS,HADS-A,and HADS-D in the observation group were significantly lower than those in the control group(P<0.05);The analysis of variance of repeated measurements showed that there were statistically significant differences in the Main effect,time Main effect,and interaction effect between time and grouping between groups(F=5.540,P=0.024;F=389.213,P<0.001;F=37.213,P<0.001).Compared within the group,the HADS-A score of the control group was significantly reduced one month after discharge compared to before intervention(P<0.05);The scores of HADS-A and HADS-D were significantly reduced 3 months after discharge compared to before intervention and 1 month after discharge(P<0.05).The scores of HADS-A and HADS-D in the observation group were significantly reduced 1 month and 3 months after discharge compared to before intervention(P<0.05);The scores of HADS-A and HADS-D were significantly reduced3 months after discharge compared to 1 month after discharge(P<0.05).(3)Comparison of scores of self-management behavior scale: Compared between groups,one month after discharge,the observation group only had significantly higher scores on the self-management behavior scale and the three dimensions of lifestyle,drug management,and safety management than the control group(P<0.05);Three months after discharge,the total score and various dimensions of the self-management behavior scale in the observation group were significantly higher than those in the control group(P<0.05);The results of repeated measurement ANOVA showed that there were statistically significant differences in the Main effect,time Main effect,time and group interaction between groups(F=228.218,P<0.001;F=526.631,P<0.001;F=389.326,P<0.001).Compared within the group,the control group showed a significant increase in scores in the four dimensions of information management,lifestyle,safety management,and seizure management one month after discharge compared to before intervention(P<0.05);Three months after discharge,the scores in both lifestyle and information management dimensions significantly increased compared to before intervention(P<0.05);The scores in the three dimensions of lifestyle,information management,and security management were significantly reduced three months after discharge compared to one month after discharge(P<0.05).The observation group showed a significant increase in scores on various dimensions of the self-management scale 1 month and 3 months after discharge compared to before intervention(P<0.05);The scores on various dimensions of the self-management scale significantly increased(P<0.05)three months after discharge compared to one month after discharge.(4)Comparison of QOLIE-31 scale scores:Compared between groups,the total score of the QOLIE-31 scale in the observation group was significantly higher than that in the control group at 3 months after discharge(P<0.05);Analysis of variance of repeated measurements showed that there was no statistically significant difference in Main effect between groups(F=0.254,P>0.05);There were statistically significant differences in time Main effect,time and group interaction effect(F=526.631,P<0.001;F=389.326,P<0.001).Compared within the group,the total score of QOLIE-31 scale in the control group and observation group at 3 months after discharge was significantly higher than before intervention and 1 month after discharge(P<0.05).(5)Comparison of postoperative complications of VNS: There was no significant difference in the incidence of postoperative complications between the two groups at discharge and one month after discharge(P>0.05).The observation group had a significant decrease in postoperative complications compared to the control group three months after discharge(P<0.05).Conclusion This study guided patients through two nursing plans,resulting in varying degrees of improvement in disease awareness,self-management behavior,and quality of life.Anxiety and depression were improved,complications were reduced,and the improvement was more significant in the cloud platform based family participation nursing group,which is worthy of promotion and use by clinical medical staff.
