Objective:To construct a structural framework for the perceived risk of the doctor-patient relationship from the perspective of medical practitioners,develop a psychometric-compliant questionnaire on the perceived risk of the doctor-patient relationship of medical practitioners,and conduct an empirical study on the level of perceived risk of the doctor-patient relationship among medical practitioners.Methods:A pool of scale entries was constructed based on the results of the literature collation,pre-survey and qualitative research,combined with group discussions and experts’advice to develop an initial questionnaire on the perceived risk of doctor-patient relationships of medical practitioners.Through convenience sampling method,the data was collected using the initial questionnaire,which was randomly divided into two samples in the ratio of 3:2.Sample 1:The items were screened by item analysis and exploratory factor analysis to form the official questionnaire.Sample 2:The reliability and validity of the questionnaire were tested by internal consistency,expert judgement method and validation factor analysis.An empirical study of the perceived risk of doctor-patient relationship of medical practitioners was conducted using the Medical Side Perceived Risk of Doctor-Patient Relationship Questionnaire.Statistical methods such as independent sample t-test,ANOVA and multiple stepwise regression were applied to explore the influence of demographic variables on the level of perceived risk in doctor-patient relationship and the relationship between each dimension and the total score.Results:The development of the questionnaire on the perceived risk of the doctor-patient relationship of medical practitioners:Through pre-survey,qualitative research,expert consultation,item analysis and exploratory factor analysis of data,the questionnaire was developed to include four dimensions(responsibility perception dimension,social connection dimension,individual psychological dimension and doctor-patient trust dimension)and 14 items.The cumulative variance contribution of the four extracted common factors was 76.133%.The questionnaire was tested for reliability and validity.The overall Cronbach’sαcoefficient for the questionnaire was0.772,with Cronbach’sαcoefficients of 0.913,0.884,0.923 and 0.903 for the four dimension respectively;the content validity index(S-CVI/Ave)was 0.939.All indicators were within the standard range(X~2/df(1.391)<3,RMSEA(0.043)<0.08,PGFI(0.636)>0.5,PNFI(0.744)>0.5,AGFI(0.911)>0.8,GFI(0.940)>0.9,CFI(0.986)>0.9,IFI(0.958)>0.9,TLI(0.982)>0.9).Single factor analysis showed that there were significant differences in the scores on the responsibility perception dimension between medical practitioners with different departments and education levels;there were significant differences in the scores on the social connection dimension between medical practitioners with different annual salaries and whether the hospital had health insurance;there were significant differences in the scores on the individual psychological dimension between medical practitioners with different marital status and type of household registration;there were significant differences in the scores on the doctor-patient trust dimension between medical practitioners with different marital status;and there were significant differences in the total scores on the perceived risk of the doctor-patient relationship between medical practitioners with different education levels,marital status,type of household registration and whether the hospital had health insurance.Correlation analysis showed that there was no significant correlation(p>0.05)between the responsibility perception dimension score,social connection dimension score,individual psychological dimension score,doctor-patient trust dimension score and the total score of perceived risk of doctor-patient relationship with department and education level.The responsibility perception dimension score was significantly and positively correlated with marital status,type of household registration and whether the hospital had medical insurance(p<0.01);the social connection dimension score was significantly and positively correlated with marital status and type of household registration(p<0.05);the doctor-patient trust dimension score was significantly and positively correlated with marital status and type of household registration(p<0.05)and negatively correlated with annual salary(p<0.01);the total score was significantly and positively correlated with marital status,type of household registration,and whether the hospital had medical insurance(p<0.05).Multiple stepwise regression analysis showed that the dimensions of responsibility perception,social connection,individual psychological and doctor-patient trust had a significant positive effect on the total score of perceived risk in doctor-patient relationship,while hospital rank had a significant negative effect on the total score of perceived risk in doctor-patient relationship.Conclusion:The perceived risk of doctor-patient relationship questionnaire of medical practitioners developed in this study has good reliability and validity,which can be used as a tool to measure the level of perceived risk of doctor-patient relationship of medical practitioners.Responsibility perception,social connection,individual psychology,doctor-patient trust and hospital rank are the main factors influencing the level of perceived risk of doctor-patient relationship of medical practitioners.
