Background: The state has issued a series of medical reform policies and taken multiple measures at the same time,so that China’s basic medical and health service system covering urban and rural areas has been more perfect,service fairness and accessibility have been significantly enhanced,and service levels and benefits have been continuously improved.In recent years,social and economic development and health care have made rapid progress,but due to people’s demand for diversified and multi-level medical services that have not been effectively met,and the imbalance in the distribution of health resources,patients are more inclined to go to large cities and large public hospitals for treatment,while primary medical and health institutions are neglected.Objective : Taking bounded rationality and endowment effect as the theoretical starting point,this study attempts to analyze and elaborate the internal psychological behavior logic that affects patients’ medical decisions,and supplement and adjust the existing research results,so as to enrich the scientific vision and research connotation in the current field of medical treatment.Furthermore,it is hoped that reasonable decision-making logic and feasible measures will be put forward to reduce the pressure of people seeking medical treatment,promote the sinking of medical resources,rationally standardize patients’ medical treatment,and form a benign and standardized medical treatment system.Methods: Through a large number of relevant literature review,expert consultation was organized,and the questionnaire was designed by himself.Using random sampling method,outpatients from different departments of a tertiary hospital in Hefei were interviewed and surveyed from September to October 2022.Epi Data 3.0 software was used for double data entry,and SPSS Statistics 26.0 software was used to analyze the data.The exported data were statistically analyzed using SPSS17.0,including factor analysis,descriptive analysis,t-test,one-way ANOVA,rank sum test,and multiple linear regression analysis.Results:(1)The descriptive statistical results showed that when choosing a medical institution,the scores of patients considering their own factors were 3.90±0.694 points,the average score of the factor dimension of medical institution was 4.55±0.543 points,and the average score of the patient’s limited rationality dimension was 3.18±0.982 The average score of the endowment effect was 3.68±0.563 points.(2)Through multiple linear regression analysis,it can be seen that gender,marital status,and cultural and educational level will have an impact on patients’ attention to their own factors.Marital status,occupation,and perceived physical condition are the influencing factors that affect the factors that affect patients ’ attention to medical institutions;The age,place of residence,education level,occupation,type of medical insurance,and perceived physical condition of outpatients are the factors affecting the limited rationality of patients.Age,place of residence,per capita monthly income,marital status,education level and perceived physical condition were the factors affecting the endowment effect of patients(P<0.05).(3)Analyzing the behavior of patients with different sociodemographic characteristics when faced with common or frequent diseases,among which permanent residence,cultural and educational level,occupation,per capita monthly household income,type of medical insurance and perceived physical condition will affect them(p<0.05),patient’s permanent residence,marital status,cultural and educational level,occupation,Monthly income per capita and type of health insurance both affect the choice of medical institution in the event of acute or critical illness(p<0.05).Conclusions:(1)Outpatients consider the factors of medical institutions more than their own factors when choosing medical institutions.At the same time,there are obvious bounded rationality and endowment effects in their medical behavior.(2)In order to promote patients’ reasonable medical treatment,relevant government departments need to strengthen policy publicity and guidance,increase financial investment,and guide the sinking of medical and health resources;Medical institutions should improve their technical level,meet the medical needs of the people,enhance service awareness,and improve service quality and efficiency;The masses need to change their concepts and habits of seeking medical treatment,and have the courage to jump out of the "comfort zone of medical treatment". |
